The County of Yolo is recruiting to fill one (1) regular, full-time position in the class of Intensive Case Manager I/II at the Health and Human Services Agency.
The incumbent will work the CalWORKs Intensive team and case managers who are both adaptable and flexible when working with multi-barrier families using Human-Center Design and a Strength-Based Approach. The goal of our programs is help our customers reach self-sufficiency by using a human-centered approach and providing quality, relevant services. We need innovative staff who are not afraid to think outside of the box, and who understand the importance of serving our vulnerable customers. This team works with families that may have multiple life challenges i.e. homelessness, child welfare involvement, mental health/substance abuse, health, disabilities (physical & learning), and lack job retention skills.
Intensive Case Managers expected to complete home visits and have constant contact with the family until the family is deemed stable. Intensive Case Managers work as a 'team' to support the families and with other teams within the CalWORKs Program, such as WTW case managers, CalWORKs Public Assistance Specialist, the Workforce Innovation & Opportunities Act, and partner agencies that have resources to support the families on their journey. Experience in any of the above areas is a plus. The level of hire will be determined by the hiring manager at the time of offer.
The ideal candidate
Would have knowledge of CalWIN and Homeless Management Integration System (HMIS), Housing First Model, and foundational knowledge of the CalWORKs Welfare-To-Work program.
The ideal candidate will be innovative and have a 'think-outside-the-box' approach; and understands the importance of serving our most vulnerable families. Be familiar with our employment & business services programs (WTW, WIOA, Employment Center, etc.)

$27.53-$33.46 hourly
$4,772-$5,801 monthly

Definition
Under supervision, performs work in a variety of social services programs designed to support clients in need of intensive case management services; assists families and individuals to overcome multiple barriers towards higher education, employment, and/or permanent housing; participates on multi-disciplinary teams to collaborate on best ways to support clients within multiple social services and behavioral health programs; and maintains positive relationships with landlords and other housing staff to ensure ongoing affordable housing options. Distinguishing Characteristics
The Intensive Case Manager class has two salary levels, and positions in this class are flexibly staffed. The Intensive Case Manager I is the entry-level classification in this classification series. Initially, incumbents work under closer supervision, and are given detailed instruction in the performance of work related to Welfare-to-Work, CalWORKS Housing Support Allocation Program, Family Stabilization program, Cal-Learn, and cases requires crossover with other Health and Human Services Agency (HHSA) programs and outside agencies. Incumbents in this class will assist clients through challenging situations such as mental health crises, homelessness, credit report concerns, landlord relations, domestic violence, substance use, basic needs emergencies and child welfare, probation, and/or parole involvement.
Initially, work is more structured and repetitive than those assigned to the Intensive Case Manager II. As skills and knowledge are developed, incumbents are expected to perform increasingly responsible and difficult assignments. Incumbents may advance to Intensive Case Manager II after demonstrating the ability to perform the full journey level duties of the class. The Intensive Case Manager II is the journey level classification responsible for performing the full range and scope of work. Incumbents work under general supervision and are expected to be proficient in performing a variety of responsible and difficult employment services work.
The Intensive Case Manager series varies from the Employment Services Specialist series in that the latter work with participants in the areas of employer engagement, career assessment, employment counseling, job search activities, training activities, job development and/or case management. This series focuses on intensive case management of participants with multiple, complex barriers to employment (housing, child welfare, substance use, etc.) where services typically overlap with other areas of the HHSA.
Essential Functions
Duties may include, but are not limited to, the following: Navigates clients through the process of obtaining multidisciplinary services through the HHSA or other community resources; receives internal and external referrals and determines individual and family needs for services; provides appropriate support to facilitate expedited access to resources.
Initiates contact with appropriate programs and resources on behalf of client in an effort to engage the client and maintain trust and rapport through to the client's next point of contact; follows up to ensure the continuity of service.
Performs intensive casework services with clients or patients and their families to improve or restore individual or family functioning and stability; develops and administers long and/or short-term plans including homeless services, mental health services, drug and alcohol services, family maintenance/reunification, etc.
Conducts initial housing assessment for participants; documents and maintains up-to-date information on services provided to participants in the Homeless Management Information System (HMIS).
Conducts research, outreach and education to, and develops and maintains relationships with, current and potential landlords and property management companies.
Acts as a positive support liaison between landlords and participants to ensure housing stability; responds to complaints from landlords and participants related to housing conditions; provides mediation as needed; disburses rent checks monthly.
Conducts weekly client visits with clients where they are most comfortable including home visits, motel/hotel visits, office visits, etc.; provides tenant education including: tenant rights and responsibilities, housing discrimination and communication with landlords.
Works collaboratively with other agencies, businesses, and co-workers to assist participants as needed during their move into permanent housing, including accessing rental assistance monies, moving costs, purchasing furnishings and other basic household items for the family, etc.
Gathers, compiles, monitors and analyzes relevant data regarding the delivery of services; identifies and addresses gaps and barriers in processes; recommends modifications and changes to work processes regarding efficiency and quality improvements; creates spreadsheets, writes reports and presents information as required.
Performs related duties as assigned.
Employment Standards
Intensive Case Manager I:
Education:
Bachelor's degree from an accredited college or university with at least 30 semester or 45 quarter units in social work, sociology, social science, counseling, psychology or other human services field; AND
Experience:
One (1) year of full time, paid experience comparable to an Employment Services Specialist I or higher with the County of Yolo; or one (1) year of full time, paid experience working with homeless families, private homeowners, property management companies on eviction prevention processes including court proceedings, landlord-tenant intervention and credit resolution.
Substitution:
Up to four (4) years of education may be substituted by relevant, paraprofessional case management experience in a health, employment or social services agency, where one (1) year of full time experience is the equivalent of thirty (30) semester units.
Intensive Case Manager II:
Education:
Bachelor's degree from an accredited college or university with at least 30 semester or 45 quarter units in social work, sociology, social science, counseling, psychology or other human services field; AND
Experience:
One (1) year of full time, paid experience comparable to an Intensive Case Manager I with the County of Yolo.
Substitution:
Up to four (4) years of education may be substituted by relevant, paraprofessional case management experience in a health, employment or social services agency, where one (1) year of full time experience is the equivalent of thirty (30) semester units.

Understand, implement and explain the agency's programs, policies, and procedures
Establish and maintain cooperative working relationships with agency staff and the general public, and provide culturally competent services
Maintain records in an accurate, organized, confidential and efficient manner
Apply critical thinking skills to be able to recognize and evaluate problems and find solutions
