Our Caregiver are responsible for:
Overall care for specifically assigned clients
Learning, implementing and maintaining the core values of Adult Transitional Care
Delivering first class service excellence to each and every Adult Transitional Care client
Communicating all scheduling changes, concerns, and any other care related issues directly to the Care Manager for each client they have been assigned
Documenting in accordance with Adult Transitional Care documentation system set forth
Studying and carrying out each “Care Plan” in detail for each client they are assigned
Caregiver Qualifications:
Demonstrate passion for helping and caring for others
Ability to effectively communicate with clients in an amicable manner
Own reliable transportation
Ability to transport client in your vehicle, or client's vehicle
Ability to pass a drug screen, criminal background check, and DMV
Possess a valid driver's license
Must have valid car insurance
