Description
The Behavioral Health Care Coordinator, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well being of members. The Behavioral Health Care Coordinator’s work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Job Description
The Behavioral Health Care Coordinator employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
This position will support Humana member’s access to behavioral health services, including a focus on the increased need in behavioral health services brought on by the COVID-19 pandemic. This position will provide member outreach and education related to behavioral health including assessing risk, determining appropriate care in individual patient communities, and collaborating with providers as well as clinicians across other Humana departments.
The Behavioral Health Care Coordinator must live in one of the following states:
CA, GA, SC, TN, AR, KS, MO, OK, IL, IA, NE, MN, MT, ND, SD, WI, WY, CT, MA, ME, NH, NJ, NY, RI, VT, MD, NC, VA
Required Qualifications
Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university
3-5 years of experience working as a behavioral health professional including working with persons experiencing serious mental illness or substance use disorder and related treatment modalities
Experience with behavioral change, health promotion, coaching and wellness
Proficiency with Microsoft Office Programs, e.g. Word, Excel, PowerPoint, Access
Must reside in one of the following states:
CA, GA, SC, TN, AR, KS, MO, OK, IL, IA, NE, MN, MT, ND, SD, WI, WY, CT, MA, ME, NH, NJ, NY, RI, VT, MD, NC, VA
Preferred Qualifications
Experience supporting patients telephonically
Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team
Current valid social service license, e.g. MSW, LCSW, LPC, LMHC
Experience working with both children and adults
Bilingual (English/Spanish)
Additional information
Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work
Must have accessibility to high speed DSL or cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance from Humana systems is 10M x 1M
Associates working in the state of Arizona must comply with the Tobacco Free Hiring Policy and upon offer will be subjected to nicotine testing as part of a 10-panel drug test
