Description

The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.

Responsibilities

Job DescriptionThe Care Coach 1 employs a variety of strategies, approaches and techniques to manage a member's health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment and data. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities, and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

The hours for this position are an 8 hour shift between 9AM and 6PM Eastern Time with occasional evening and weekend work required to support business needs.

This position maintains contact with providers, facilities, and members to check on status and manage member transitions. This position will document and research in multiple systems. This position will print and mail educational information to members.

Required Qualifications


Bachelor's degree
Proficient in MS Office products
Experience with documenting in multiple computer applications/systems at one time
Prior healthcare industry experience
Telephonic communication experience in a professional environment
Exceptional verbal/written communication and interpersonal skills
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Comfortable working in a routine environment
Comfortable guiding difficult conversations
Must have a separate room with a locked door that can be used as a home office to ensure you have absolute and continuous privacy while you work
Must provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 25 Mbps download and 10 Mbps upload is required


Preferred Qualifications


Master's Degree
Previous experience with electronic case note documentation
Bilingual Spanish/English
