Responsibilities

Conviva Care Solutions is seeking a hard-working and dedicated Manager, Care Management who may lead and supervise a clinical and non-clinical team of nurses and who is interested in being part of a team that focuses on excellent service to others.

Job Functions:


Leads the care coordination team in the market
Assist with hiring, training, coaching, developing a highly functioning clinical team in the market
Monitor workflows, business trends, outcomes and metrics.
Works collaboratively with the Market Vice President to help set strategy and tactics for the market care coordination
Works closely with the market physician team
Collaborate with health care professionals in reviewing actual and proposed medical care and services with a goal of improving member's lifelong wellbeing while achieving organization's goals.


Education:


Valid/current RN License
BSN or Bachelor's degree in a related field


Qualifications/Requirements:


Minimum of 3 years prior clinical leadership experience
Possess ability to communicate well both telephonic and face to face
Ability to travel to orient new associates onsite

Previous experience in utilization management within Insurance industry
Previous Medicare Advantage/Medicare/Medicaid Experience a plus
Previous experience in Front End Review, SNF, LTAC, DME or Home Health.
English and Spanish bilingual preferred
