This staff Registered Nurse ED Case Manager position is located in the Emergency Department at the Robley Rex VA Medical Center, Louisville, Kentucky Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Case management, emergency department and Geriatrics experience not required but preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: You must be able to perform work that requires regular and recurrent bending, lifting 30 pounds or more, stooping, climbing, twisting and/or bending, standing and/or walking almost constantly during the course of the day, reaching above the shoulder, stretching, and similar activities supporting various weights; walking and standing for 8 hours; frequent pushing medication carts, wheelchairs, litters, beds; use of fingers, both hands and both legs; emotional stability (you must be able to care for Veterans with various medical and psychiatric illnesses who at times may exhibit unpredictable behavior and may have the potential to be physically or verbally assultive); near vision correctable at 13 to 16 / far vision correctable in one eye to 20/50 and to 20/100 in the other / both eyes required with ability to distinguish basic colors; hearing aid permitted in order to sufficiently hear various alarms and respond immediately. [The Nurse ED Case Manager is a licensed registered nurse who meets the educational, experiential, and competency requirements defined in the Nurse Qualifications Standards. He/she assesses, plans, implements, coordinates, evaluates and collaborates with the interdisciplinary team, patients, families and community providers. He/she serves as an integral member of the treatment team and is responsible for helping to produce a therapeutic environment and plan of care to promote optimal health status. He/she screens Geriatric ED patients for alternate level of care options and provides education to patients, caregivers and staff on those options for care. He/she facilitates discharge planning for patients that do not require an acute hospital admission from the ED setting. He/she assures that all patients that are not assigned to a PACT are provided education as to that process and followed-up by a designated PACT per patient agreement. He/she collaborates with social work, PACT and nursing service to facilitate acute rehabilitation admission, transfer to a skilled nursing facility, initiation or resumption of skilled/nonskilled HHC services and referral to other outpatient providers and VA Programs. The Nurse ED Case Manager demonstrates expertise in leadership and initiative in delivering and improving holistic care through collaborative strategies with others, ensuring compliance with the mission, vision, and values of the medical center and/or VISN. The Nurse ED Case Manager applies case management principles to both Geriatric and non-Geriatric patient population to ensure appropriate utilization of resources and level of care. The Nurse ED Case Manager facilitates ED patient flow by identifying and resolving barriers which may hinder effective patient care. He/ she engages in process improvement activities that are focused on efficient care delivery and promote positive clinical and quality outcomes including reducing unnecessary admissions, readmission rates, unnecessary ED visits and time spent in the ED . The Nurse ED Case Manager concurrently plans for ED post discharge care and appropriate Geriatic follow-up when indicated.. He/she supports the successful transition of care by follow up with the patients in order to assure that the patient understands and has the tools to follow prescribed discharge plan as well as follow-up with the primary care provider when issues or concerns are identified. He/she also attempts to follow up with patients that leave prior to being seen or AMA. Patient Care area is located in the Emergency Department setting.
