Under the supervision of the Coordinated Care Director, the Discharge Coordinator, as part of an interdisciplinary team, including the patient/family, physicians, nurses, therapists and payors, ensures that the patients progress in the acute episode of care through post discharge and is quality driven while being efficient and cost effective. Applies knowledge and skills in evaluation, planning, implementation, information, referral, education, and advocacy. Maintains current knowledge of regulatory requirements and related programs and ensures patients are informed of rights and responsibilities as participants in these programs. Displays professional, safe conduct in all interactions. Maintains positive working relationships with staff, other departments, physicians and peers. Displays conduct in support of CHI St. Luke’s Health Memorial Mission & Vision.

The Discharge Coordinator identifies high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, care management, and referrals to appropriate resources.
Develops short term intervention plans consistent with patient needs, strengths, diversity and psychosocial factors as directed by the RN Case Manager.
Addresses the needs through a collaborative approach with the healthcare team.
Develops a holistic approach to any barriers or problems.
Promotes family support within the context of multicultural understanding and competency.
Provides services to maximize opportunities to the patient and family.
This role ensures that patients are assisted to achieve their highest level of function and return to the least restrictive environment post hospitalization.
Assists patient and family to gain access to formal and informal community resources.
Utilizes research and technology to assist patients and families.
Promotes community services within the context of multicultural understanding and competence to enhance family support.
Advocates and facilitates change that effectively responds to the needs of the patient.
Consults with the healthcare team on behalf of the patient and family
Interacts with patients, family members, healthcare professionals, and community and state agencies in this effort. The Discharge Coordinator serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information
Works collaboratively with the RN Care Manager, Social Worker and care team to ensure patient needs are met and care delivery is coordinated across the continuum.
Initiates and supports activities to overcome barriers and gaps in services available to patient and family to enhance the health, social and emotional well-being.
Works with internal and external individuals, groups or organizations that support and enhance the well-being of the patient and family.
Promotes collaboration among community health and mental health services to facilitate access to these services.
Uses extensive knowledge of community resources to facilitate the provisions of community services.
Effectively plans, implements, and evaluates programs that promote patient and family discharge successfully.
Applies knowledge of environmental factors in coordinating discharges.
Plans are coordinated to promote a safe and healthy discharge.
Implements the plan in collaboration with the patient, family and health care team.
Evaluates the plans to ensure that all aspects of the patient’s needs are included.
Demonstrates the judgment and initiative to operate independently and autonomously in the development, implementation and follow up of post-acute plans with minimal guidance from the department manager
Advocates, facilitates and contributes by taking accountability for outcomes aligned with patient and family well-being.
Maintains accurate documentations and records.
Performs duties and tasks in accordance with performance standards established for the job.
Is responsible for participation in and completion of all patient safety initiatives appropriate to the position.
Maintains current knowledge of federal and state laws and regulations and abides by said laws and regulations with emphasis on persons with disabilities, child welfare, mental health, confidentiality, and patient and family rights.
Organizes times, resources, energy and workload in order to meet responsibilities.
Evaluates own practice and disseminates the finding to co-workers, director and community in order to maximize effectiveness of services and resources provided.
Participates in appropriate professional development activities both internal and externally to the facility to improve knowledge and skills.
Demonstrates accountability for timely and accurate prioritization, planning and preparation to assure safe discharges and cost effective utilization of available resources for the continuation of patient care.

Education Level
Minimum:
Graduate of an accredited nursing vocational program
Preferred:
Associate’s Degree in Nursing or
Bachelor’s Degree from a regionally accredited college or university in one of the following: Nursing or Social Work.
Licensure/Certification
Minimum:
Licensed in the state of Texas as a Licensed Vocational Nurse (LVN) through Texas Board of Nurse Examiners (BNE) or
Licensed Baccalaureate Social Worker (LBSW) through Texas Department of State Health Services (TDSHS) &
BLS certification (through an approved AHA program)
Preferred:
Licensed in the state of Texas as a Registered Nurse (RN) through Texas Board of Nurse Examiners (BNE) or
Licensed Clinical Social Worker (LCSW) through Texas Department of State Health Services (TDSHS)

Experience
Minimum:
