Description

The Social Services Care Manager, in a telephonic environment, assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members.

Responsibilities

The first 2 weeks of training will be from 8:30am-5pm EST. After training, the hours will be 9:30am - 6pm EST.

The Care Manager, Telephonic Behavioral Health 2 is a master level social worker and functions as part of an interdisciplinary care team receiving referrals to assist members with identified Social Determinant of Health needs. In a telephonic environment, they assess and evaluate member needs related to their physical, environmental, and psycho-social health issues. This includes completing required assessments and determining appropriate interventions such as education, linkage to community resources, and advocacy. They may create and update member care plan and complete documentation in the member's record to reflect outreach attempts/contacts with members, resources, providers, and other members of the interdisciplinary care team.

Required Qualifications


Master's degree in Social Work from an accredited university
Licensure in Master's degree field of study required
Must have passed Master, Advanced Generalist, or Clinical ASWB Exam
Minimum 3 years of experience working as a licensed Social Worker
Capacity to manage multiple or competing priorities including use of multiple computer applications simultaneously.


Preferred Qualifications


Experience working with the geriatric population
Knowledge of community health and social service agencies and additional community resources
Additional licensure in LA, MI, MS, NV, NY and OK


Work-At-Home Requirements

To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:


At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested
Satellite, cellular and microwave connection can be used only if approved by leadership
Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.
Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
