The Care Management Support Professional 1 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Professional 1 work assignments are often straightforward and of moderate complexity.

Humana's Primary Care Organization, CenterWell Senior Primary Care, is seeking a Care Coordinator who contributes to the administration of care management. This role is a key resource and member of the Ancillary Care team. The Care Coordinator provides individualized assistance to patients and is often the first point of contact before engaging with other Ancillary Care team members. The main responsibilities of the role are:
Review and triage internal referrals for appropriate Ancillary Care team resources and care management program placement
Completes intake assessment to identify appropriate resource or program placement
Follows assigned patients to ensure progressing towards desired outcomes and engages the PCP and/or Ancillary Care team members as appropriate
Manage short term and low acuity care management program caseload of patients which include coordination of care, patient education, identification of barriers, and coordinate with other team members as identified
Assist RN Care Coaches with task management and support patient care plan goals
Review care management program tasks and care alerts for the Ancillary Care team and create necessary tasks and/or addresses
Identify and address patient barriers to care by utilizing local, state or federal resources and/or referral for Ancillary Care team support
Provides care coordination for identified critical resources and helps navigate and educate on health plan benefits and/or resources
Provides general health/disease education information and/or supports educational classes related to self-management skills
Facilitate and schedule patient appointments with other care team members and PCP as appropriate
Provide support and/or participate in practice level ITMs and huddles with various levels of team members and leaders to improve patient outcomes
Required Qualifications
Clinical related or healthcare related field
Ability to multi-task and work in a fast paced environment with changing priorities
Effectively meet and exceed customer needs; build productive relationships with physicians, internal customers, and takes responsibility for customer loyalty and satisfaction
Work effectively and cooperatively with other members of the CDO Team; establishing and maintaining good working relationships
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas
Follows established guidelines and procedures
Demonstrate interpersonal/verbal communication skills
Preferred Qualifications
MA, LPN, Associate or Bachelor's degree in relevant field or equivalent work experience
Excellent time management skills
Proficiency with EHRs, or other health care related software platforms
High degree of independence, flexibility, initiative and commitment
Additional Information:
This position will work both at home and in the centers.
