Responsibilities
The Community Management Professional 2 builds visibility and credibility of organization's products and services throughout community via chats, posts, and interactions.
Strategizes methods to scale communities and drive greater connections among community members.
Engages new community members.
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Support members to navigate the healthcare system and the relevant social services.
Serve as a liaison between the member and health system.
Assist in addressing members' social determinants of health and engage members in chronic disease self-management services.
Perform home visits and community activities and identify barriers to care and drive member activation and empower members to engage in their care and adopt new healthy behaviors.
Identify barriers in the community and access to the necessary services.
Developed strong partnership with community-based organizations.
Conduct initial barriers to care screenings and collaborate with care management team to engage patients in the appropriate programs.
Assist members and their families with qualified benefits, related to social, physical, behavioral and emotional wellbeing needs.
Maintain documentation and compile statistical data according to departmental guidelines.
Follows established guidelines/procedures.
Required Qualifications
Associates Degree in business, communications, or related field or equivalent work experience is required.
Minimum of two (2) years of experience in community outreach and/or marketing and sales.
Excellent written and verbal communication skills.
Intermediate to advanced computer skills and experience with Microsoft Word, Excel and Outlook.
This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Must have a separate room with a locked door that can be used as a home office, to ensure you have absolute and continuous privacy while you work.
Must have the ability to provide a high speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required.
Must be passionate about contributing to an organization focused on continuously improving consumer experiences.
Must reside in the state of Oklahoma; travel within the state is expected (See Additional Information section for information on travel) .
Preferred Qualifications
Experience in Managed Care or Health Insurance.
Prior experience with Medicare & Medicaid recipients.
Knowledge of community health and social service agencies and additional community resources.
Bilingual preferred (Spanish, Native Tribal, Vietnamese or other).

Typical Days/Hours : Monday - Friday; 8:00am - 5:00pm Central Standard Time (CST)
Days/Hours: Monday - Friday; 8:00am - 5:00pm Central Standard Time. May require to work weekends (Saturdays) occasionally for community engagement activities.
Travel: 75-80% within the state of Oklahoma to meet with members, community leaders, attending and coordinating events. Occasional travel to our Oklahoma City office location for staff meetings, training, etc.
Interview Format

If you are selected to move forward from your application prescreen, you will receive correspondence inviting you to participate in a pre-recorded Voice Interview and/or an SMS Text Messaging interview. If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
