\u003cp\u003eThis position will oversee two programs in Stockton, CA.\u0026nbsp; \u003cstrong\u003eTelecare San Joaquin Connect I\u003c/strong\u003e serves adults within a high -fidelity ACT model.\u0026nbsp;\u003cstrong\u003eTelecare San Joaquin Connect II \u003c/strong\u003eis\u0026nbsp;a program serving adults with justice system involvement. These new Full Service Partnerships in the Stockton area which will serve adults being discharged from BHS 24‐Hour Services and\u0026nbsp;associated inpatient hospitals (including out‐of‐county placements) who are disconnected from mental health system and have minimal knowledge of how to access resources considered at high‐risk of symptom re‐emergence or\u0026nbsp;readmission, eligible to participate in FSP‐level services. Each program will provide outpatient services to 50 members.\u003c/p\u003e

\u003cp\u003eStaff will do \u0026ldquo;whatever it takes\u0026rdquo; to improve health and social outcomes for clients, including identifying linkages to ongoing mental health services and other relevant resources.\u0026nbsp;Wraparound services and supports will not only include clinical interventions, such as multidisciplinary assessment and treatment planning, 24/7/365 crisis intervention, medication support, and integrated co-occurring substance abuse interventions, but also non-mental health support in such areas as community integration, housing, employment and education, transportation, and benefits. Peer Recovery Coaches provide consultation to the team to promote and reinforce Telecare\u0026rsquo;s Recovery Culture as defined by the Telecare Recovery Centered Clinical System (RCCS) in which each member\u0026rsquo;s point of view and preferences are recognized, understood, respected, and integrated into services and self-help programming. Additionally, Peer Recovery Coaches respond to critical situations with high-level engagement and deescalation skills which support a least restrictive environment for members served experiencing an emergency related to a mental health/addiction challenge. \u003c/span\u003e\u003c/p\u003e
