The Patient Service Manager's responsibilities are to act as a clinical resource person for agency staff and administration, and evaluate patient care services provided by the nursing staff employed directly by or through

contract with the CHHA.   Additionally, you will resolve operational issues that promote patient satisfaction.

DUTIES INCLUDE:

*Guides staff to implement the home care nursing role according to Joint Commission, NYS DOH, CMS standards *Participates in ongoing nursing and clinical interdisciplinary staff competency assessment evaluations *Consults with individuals/department to assist with attainment of education/professional goals *Provides general Home Care orientation for interdisciplinary team members and support staff *Evaluates performance of new nurses at the close of orientation and the probationary period

Performs audits and surveillance activities as part of ongoing survey readiness

Participates in all survey readiness activities

*Suggests and implements quality improvement activities to maintain high quality care *Monitors occurrences, complaint and concern reporting and coordinates the response as it relates to customer service

KNOWLEDGE, SKILLS, & ABILITIES:

*Knowledge of clinical documentation protocols, home health care regulations and standards, and home health care fiscal parameters *Current clinical knowledge regarding pathophysiology, current disease treatment modalities and drug therapy parameters *Ability to communicate successfully both orally and in writing, including effective listening skills *Ability to interpret agency goals, policies and procedures, job functions and performance requirements

REQUIREMENTS:

*License and current registration to practice as a Registered Professional Nurse in New York State *Baccalaureate Degree in Nursing from an approved program required. Master's Degree preferred. OASIS Certification preferred. Home Health Coding Certificate preferred. *Two (2) years' experience as a community health nurse with a Certified Home Health Agency *One (1) year experience in the field and one (1) year supervisory experience preferred, or an equivalent combination of education and experience *Must have valid driver's license and ability to provide own transportation during working hours for the purpose of making home visits
