Description
The Care Management Support Assistant 2 contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Management Support Assistant 2 performs varied activities and moderately complex administrative/operational/customer support assignments. Performs computations. Typically works on semi-routine assignments.

Conviva is seeking a dedicated and compassionate - ACCESS Representative, who is interested in being part of a team that focuses on excellence and service to others.
Essential Functions:
Addresses and document inquiries by following department process and Department of Children and Families (DCF) guidelines.
Serves as advocate and educator to applicants in need of assistance of services provided by DCF.
Comply with all HIPPA and rules and regulations.
Maintain applicant files and follow up on case status.
Provide weekly and monthly report to direct supervisor.
Contribute to team effort by accomplishing related results as needed
Required Qualifications
Bilingual English and Spanish
Knowledge of Microsoft Word, Excel and Outlook
Administration support experience in a healthcare industry
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Strong customer service skills
Attention to detail
Effective verbal and listening skills
Ability to multi-task, prioritize and manage time effectively.
Preferred Qualifications:
High School Diploma
Associate's Degree
Microsoft Access proficiency
