Description

Humana Healthy Horizons in Louisiana is seeking Behavioral Health Liaisons to join our growing team. The Behavioral Health (BH) Liaisons will be responsible for ensuring connection, coordination, outreach, education, and community involvement for the education systems, department of children and family services system, court systems, and law enforcement. The BH Liaisons will also work to ensure members have access to and are aware of services available through the Local Governing Entities (LGEs) and that effective medical and behavioral services are in place for the Intellectually and Developmentally Disabled population. Responsibilities

The Behavioral Health Liaison will work in a variety of areas:

Child Serving Entities

Liaise between the Health Plan and the State's child serving entities (LA Dept of Education (LDOE), Office of Juvenile Justice (OJJ), and Department of Children and Family Services (DCFS)) to ensure the best possible care of members.

Work with and serve as point of contact (POC) for regional DCFS offices to support and improve health outcomes of Louisiana's most vulnerable children and youth.

Outreach to school systems across the state to educate on the services available for children through the health plan.

Have knowledge of and provide education on the entire behavioral health service array, including CSoC, crisis services and process for obtaining services and out of home placements and process for placement.

Serve as a point of contact for judges, court personnel and appearing in court when requested by the court system or LDH.

Serve as a point of contact for LDH legal and staff working with LDH custody cases.

Collaborate and intervene as appropriate on matters pertaining to drug court, juvenile court, family court and criminal court processes and issues.

Provide continuous outreach and education to the judicial system on access to services.

Coordinate and consult with internal departments, providers, and community organizations to support the delivery of high-quality services to vulnerable children and youth populations.

Local Governing Entities

Liaise between the Health Plan, the Local Governing Entities (LGE), and the Office of Citizens with Developmental Disabilities (OCDD) to ensure members are connected and receive available mental health, addictive disorders, and developmental disability services.

Collaborate and intervene as appropriate on matters pertaining to addictive disorder services, mental health services, gambling, suicide prevention, and the range of developmental disability services available to members.

Serve as the primary point of contact for LGEs and OCDD to address any inquiries and/or barriers, to resolve any issues, and/or provide a warm hand-off to the appropriate Health Plan's team member.

Provide continuous outreach and education to LGEs and OCDD to ensure awareness of the Health Plan's benefits and services to support its members in accessing services and supports.

Serve as SME for internal teams and support the delivery of training related to supporting members in accessing services of LGE and OCDD.

Attend Human Services Interagency Council (HSIC) meetings, as required and requested by LDH.

Coordinate and consult with internal departments, providers, and community organizations to support the delivery of high-quality services to vulnerable populations.

$5000 SIGN-ON BONUS for new associates! $3000 payable within the first 45 days and the remaining $2000 after 1 year of service.

Required Qualifications

Licensed Mental Health Professional with one of the following licenses: Licensed Masters Clinical Social Worker (LCSW) OR Licensed Professional Counselor (LPC) OR Licensed Marriage and Family Therapist (LMFTs) OR Licensed Addiction Counselor (LAC).

Five (5) years of experience working with children, adolescents, adults, or families experiencing behavioral health and/or developmental disabilities.

Experience navigating the state's safety net behavioral health program and services through LGEs.

Ability to work in a variety of settings with culturally diverse families and communities, and the ability to be culturally sensitive and appropriate.

Experience with Microsoft Office Suite, including Word, Outlook, and Excel.

Experience utilizing a variety of electronic information applications/software programs including electronic medical records.

Exceptional communication and interpersonal skills with the ability to quickly build rapport.

This role is a part of Humana's Driver Safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:

At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested.

Satellite, cellular and microwave connection can be used only if approved by leadership.

Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.

Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.

Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Preferred Qualifications

Knowledge of community resources and behavioral health practices with high-risk populations.

Knowledge of the non-Medicaid uninsured system.

Knowledge of the I/DD service delivery system.

Prior case management, school experience, or experience working in the courts.

Additional Information

Workstyle: Home. Home workstyle is defined as remote but will use Humana office space on an as needed basis for collaboration and other face-to-face needs.

Location: must reside in Louisiana

Travel: up to 30% within the state of Louisiana for meetings with providers and state entities.

Typical Work Days/Hours: Monday through Friday; 8:00am - 5:00 pm (CST)

Interview Format
