Job Summary and Scope
Provide safe, direct patient care utilizing the nursing process in the ambulatory setting under direction of the Director and Executive Director of Ambulatory Clinics and the Chief Nursing Officer (CNO), to plan and care for a specialized group of patients. Provide clinical leadership for staff and oversight of ancillary staff, when designated. Serve as a role model to promote the SRMC Values, a positive work environment and efficient, quality patient care. Ensure adherence to health system, hospital and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Essential Functions:
POLICIES, PROCEDURES AND GUIDELINES- Maintain established hospital as well as departmental policies and procedures, objectives, and quality assurance programs
PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
Report potential and actual patient safety, concerns, medical errors and or near misses in a timely manner thru the chain of command and document within the PSN system.
Establish a compassionate environment by providing emotional psychological and spiritual support to patient, friends and families.
Promote patient’s independence by establishing patient care goals; teaching patient, friends, identified ley caregiver and family to understand condition, medications, and self-care skills; answering questions.
Maintain a cooperative relationship among health care teams and resolve patient problems and needs by utilizing multidisciplinary team strategies.
Active participation in hospital strategic initiatives such as quality improvement and monitoring TJC compliance.
Participate in orientation and evaluation of new personnel.
Assist with or institute emergency measures for sudden, adverse developments in patients care.
Assist provider with special tests, procedures and complex treatments.
Ensure that equipment is maintained; monitor supply and medicine requisitions; maintain supply stock levels in accordance with patient care needs and the monthly budget.
Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand.
Provide and document nursing care. Ensure that provider's orders are carried out; administer medications and perform routine treatments as ordered.
Triage patient care calls that have escalated per guidelines, document advice and follow-up calls in the E.H.R. accordingly.
Responsible for continuous learning, improvement, and evidence-based practice.
Act as liaison between patient and provider and support patient satisfaction.
Perform infusion, administer medications, and perform routine treatments, lab draws, port access, flushes and dressing changes. (Infusion RN ONLY).
Adhere to and promote the core expectations of the Patient Centered Medical Home (PCMH RN only).
Track and report statistics on immunizations, injuries, and exposures to leadership and designated committees (OHS RN only).
Coordinate all staff immunization campaigns for the hospital (OHS RN only).
Record and report pertinent observations and reactions regarding patients to medical staff.
Perform miscellaneous job-related duties as assigned.

Education/Experience
Education:
Nursing program (nationally accredited) graduate
Licenses/Certifications
Licensed Registered Nurse (RN) in State of New Mexico
CPR Certification or must obtain within 30 calendar days of date of position;
Cardiology Clinic RN
must obtain ACLS with in 30 calendar days of position.
Work Experience:
Two (2) years clinical work experience.
Preferred Educational/Experience Requirements:
Bachelor’s Degree in Nursing, AAACN certification preferred; Three plus (3 ) years Ambulatory Clinic experience in relevant specialty preferred.
Language:
Bilingual / English-Spanish preferred.
Supervision
Job is supervisory to the extent that daily work direction is provided to personnel in subordinate classifications.
Conditions of Employment
Must pass a pre-employment criminal background check, reference checks and a post offer drug screen.
Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).

Working Conditions
Typical office and/or patient care, acute care hospital environment.
Must be able to travel locally between facilities and within the surrounding community.
Occasional exposure to minimal physical risk

Involves moderate exposure to physical risks, i.e., extreme exposure to chemicals, dangerous equipment and/or materials requiring basic safety precautions
