Description
The care team clinical pharmacist is responsible for providing comprehensive, relationship-driven pharmaceutical care for members in order to support optimal health and well-being. Clinical activities will include comprehensive medication reviews, provision of drug information services, chronic disease management and education, participation in cost-reducing and quality-enhancing activities, and supportive relationship building through multichannel interactions with patients, caregivers, and the surrounding healthcare community.
Responsibilities
The care team clinical pharmacist will:Serve as a medication expert and clinical resource for the multi-disciplinary care team *Assess patient needs related to medication therapy and deconstruct barriers to care through relationship building and evaluation of member profilesAssist members and caregivers in identifying, measuring, tracking, and attaining whole health goalsSupport pharmacy-related CMS Stars quality measures for an assigned panel of membersServe as a patient advocate by promoting safe, evidence-based, cost-effective use of drug therapyConduct medication reviews with a comprehensive vision of safety, cost, convenience, and efficacyUse population health analytics to identify populations at risk and collaboratively develop strategic initiatives to ensure continuous improvement in medication management, quality outcomes, and value-based prescribing for membersProvide drug information and clinical consultations to members, their caregivers, and care team colleaguesProvide education regarding medications and disease states to both individual members and in group settingsCollaboratively design, implement, and monitor evidence-based actionable care plans that are aligned with the member's life goalsPromote an environment of teamwork and collaboration by establishing strong personal relationships with members, partners, providers and Edge care team colleagues*Document all clinical activities and interventions with the support of technology
Required Qualifications
Graduation from an accredited School of Pharmacy with a BS Pharmacy or Doctorate in Pharmacy (PharmD)
Active and unrestricted pharmacist license in good standing within all licensed states
Completion of an ASHP accredited PGY-1 clinical pharmacy residency program prior to start date where at least 50 percent of the experience includes the provision of direct patient care services with interdisciplinary teams or three (3) years of relevant clinical experience
Experience in medication therapy management and/or chronic disease management
Experience in navigating medication cost concerns
1 years of experience working in a start-up healthcare environment
Knowledge of Federal/State laws/regulations regarding delivery of pharmacy services and the scope of practice of a pharmacist
In-depth knowledge of pharmacotherapy and pharmacy practice; experience with current outpatient medication management guidelines for chronic conditions such as diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, and congestive heart failure
Knowledge of and experience with critically evaluating medical literature and various sources of drug information
Excellent organizational and communication (verbal & written) skills
Ability to work collaboratively in a team-oriented environment
Demonstrated ability to extract pertinent information through patient interviews and/or medical records in order to reach safe and effective clinical decisions
Ability to prioritize multiple tasks, anticipate & solve problems
Strong attention to detail
Ability to identify opportunities for innovation and contribute toward overarching strategic initiatives
Competent at intermediate level or higher with MS Office programs such as Word, Excel, PowerPoint
Takes personal responsibility for remaining up to date on relevant clinical pharmacy topics
Ability to manage a large panel of members with varying levels of clinical need; eager to work as part of an interdisciplinary team and to collaborate closely with other disciplines

Preferred Qualifications
Doctorate in Pharmacy (PharmD) or other advanced pharmacy degree
Licensure in MI, TN, LA, or GA.
Certification as a Diabetes Educator (CDE) or strong interest in pursuing
Board Certification in Ambulatory Care Pharmacy (BCACP) or geriatric pharmacy (BCGP) or strong interest in pursuing
Clinical residency experience with a focus in ambulatory care pharmacy or geriatric pharmacy
2 years working in an ambulatory care/primary care practice setting
