urrently seeking an experienced and motivated Assistant Team Leader for our 

ACT

 Team. This position 

shares in a leadership role

managing a multi-disciplinary team that includes case managers, specialists, nurses and clinicians.

Leads in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health 

management

 and prevention activities. 

Working in collaboration with the team leader, ACT staff 

and other staff personnel to 

ensure the clinical and administrative 

duties

 of the team promote effective treatment 

to clients.

Additional duties include p

rovid

ing 

counseling and case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community.

EDUCATION AND TRAINING REQUIRED:

· Master’s degree in a human 

service-related

 field required.

· Licensed or working towards licensure (i.e., LCSW/LICSW, LMHC, LCDP)

· Two years of experience in a community mental health center preferred.

· Ability to participate as a member of a multi-disciplinary team

· Approved driving record with ability to provide transportation and home visits.

· Supervisory experience preferred.
