SUMMARY
*Grant funded and co-terminus with funding*
Under the supervision of the Lead Community Health worker, provides community service navigation for eligible beneficiaries. Responsibilities may include a combination of some or all tasks depending on the program including patient screening for program enrollment, evaluating needs of patients, referring patients to community resources, coordinating appointments, providing help with enrollments with diverse resources in the community, patient follow-up and data collection. Utilizes a variety of information systems to schedule patients and process related patient record information; performs a variety of associated general clerical functions. Demonstrates outstanding customer service and the key behaviors, outlined in the Yale-New Haven Hospital core success factors and standards of professional behavior.

Responsibilities
1. Approaches patients and conducts screenings for acceptance into program(s)
2. Provides patient navigation services to patients enrolled in navigation programs
2.1 Schedules and coordinates patient referrals to community resources and appointments
3. Maintains accurate patient records and processes client records appropriately
3.1 Ensures that all necessary patient information is documented in patient files
4. Conducts follow-up with patients as needed for program evaluation needs
Qualifications
EDUCATION
High school diploma or GED required
EXPERIENCE
At least 1 year experience in health care or human service setting with relevant experience in community health education and/or outreach position. Experience working in the greater New Haven community preferred. Excellent oral and written communication skills. Bilingual Spanish, and Arabic preferred.
SPECIAL SKILLS
Ability to work independently with support and supervision from Screening Referrals Specialist, Lead Patient Navigator and/or Program Supervisor as necessary. Must be able to work collaboratively and communicate effectively with others, including patients, their families, staff, community health care providers and community partners. Candidate must be culturally competent and be able to thrive in a moderately paced, complex, health care environment where the Community Health Worker works as a key, valued member of the multidisciplinary team. Must have a working knowledge of the Greater New Haven Community including transportation modes, streets and intersections and community service locations.
PHYSICAL DEMAND
This position requires travel around the greater New Haven community to various client sites and health centers.
