Established Assisted Living and Memory Care community in Brainerd, MN is accepting applications for a RN, Case Manager to join our team.
Full-time opening with on-call rotation.
POSITION PURPOSE: The Case Manager is responsible for client care by ensuring the resident's needs are met through assessments and reassessments of the residents, development and implementation of appropriate care plans, monitoring of residents' services, and the supervision of assisted living staff. Position will provide direct nursing services to clients, thereby assisting each resident to reach and maintain his or her highest level of performance and independence.
PRINCIPAL ACCOUNTABILITIES:\uf0b7 Responsible for implementing the nursing process, including assessment, diagnosis, planning, goal setting, implementationand evaluation, in the delivery of professional and paraprofessional services for residents in senior housing.\uf0b7 Meet with the resident and family to accurately complete all pre-screening and admission paperwork, explaining the care plan and resident\/family responsibilities.\uf0b7 Assures residents have up-to-date care plans that address their needs and preferences, including interventions to address any areas of vulnerability and individualized medication management, treatment or therapy management plans. Reassesses the resident and modifies the care plan if necessary based on a change in condition or change in service.\uf0b7 Monitor resident's funding sources to assure that prior authorization is obtained and transferred accurately and timely to the Prior Authorization Department and that the appropriate payors are billed, when appropriate. Work with building manager to ensure all necessary authorizations are obtained in a timely manner.\uf0b7 Provide case management for residents, including the involvement of additional services, such as hospice, homecare, and transitions to the hospital, nursing home or other facilities. Participate in care conferences as needed.\uf0b7 Supervise and coordinate maintenance of appropriate, accurate and complete documentation of information on clients and staff under your supervision according to agency policy.\uf0b7 Market the wide range of health care services to potential clients and their families.\uf0b7 Participate in care conferences to provide nursing and home health expertise for client discharges as needed.\uf0b7 Assist in orienting, training, competence assessment and day-to-day scheduling of qualified staff to ensure appropriate delivery of client services.\uf0b7 Provide direct and indirect supervision and management of paraprofessional and other professional staff providing home health services to residents to assure delivery of high quality care; re-train, evaluate or correct employees' performance according to established criteria and policies.\uf0b7 Assures that residents' medication management and records are up to date, medication administration procedures are followed properly, investigate any errors and issues with staff, and implement corrective action as needed. Work with pharmacies to ensure accurate medications are received and medication reconciliation is current.\uf0b7 Assures that each assisted living staff person is oriented to the individual needs of each resident the person will serve and that staff clearly understands and follows all client care procedures. Delegate appropriate nursing functions according to State Licensure and agency policies and procedures.\uf0b7 Assist in workers' compensation process as necessary to complete timely claims.\uf0b7 Maintain compliance with current county, state and federal regulations affecting health care.\uf0b7 Actively participate in agency-wide activities and its continuous quality improvement program. Responsible for evaluating the care program's level of excellence and implementing process improvement strategies as indicated.\uf0b7 Respond to questions and calls from employees, clients, clients' families, doctors, outside case managers, etc. regarding problems, concerns, or general information regarding clients.\uf0b7 Prepare reports to meet predetermined schedules and actively participate in scheduled managerial and staff meetings.\uf0b7 Perform other functions as deemed appropriate by the Management or Executive Team.

QUALIFICATIONS:
Current licensure as a Registered Nurse in the State of Minnesota. Bachelor's Degree in Nursing preferred, but not necessary.
Preferably, a minimum of two years' experience in nursing in health care. Prior supervisory experience preferred.
Position requires candidates to have a high level of proficiency in general computer programs including Microsoft Excel and Word.
Candidates will be subject to a criminal background study and must be able to meet the physical requirements of the position.
