Come join our Team!! We are currently seeking a Case Manager to function as a Hospital Liaison for one of our integrated health home teams in our Community Support Program. You will assist with the transitional care of clients who have been hospitalized by coordinating discharge plans and person-centered recovery goals with Hospital Discharge Planners/Social Workers. The Hospital Liaison will maintain ongoing communication with all team members, hospital staff and ancillary providers while providing continuous support to identified clients until crisis has been resolved.

The successful candidate will:

-have the ability to work with a diverse population

-can be effective both autonomously as well as on a multidisciplinary team

-have excellent documentation skills within an electronic health record

The liaison will also carry a small caseload of clients who may require more support to decrease hospital and ED recidivism. As the case manager, you will be required to assist with the development of person-centered recovery planning with your clients. Your support and coordination will assist your clients with working towards purposeful and meaningful lives.

Education/Licensing:

Bachelor's degree in Human Services

preferred, minimum AA with at least 2

year's experience

with community mental health working with individuals with severe and persistent mental health issues

. Must have a valid driver’s license, auto insurance and reliable transportation.
