This is a responsible, non-supervisory position requiring familiarity with various government programs, such as Texas Administrative Code (TAC), Administration for Community Living (ACL), Health and Human Services Commission (HHSC), Older Americans Act (OAA), and Office Area Agency on Aging (OAAA) and insurance policies regarding aging programs and services and other AACOG programs. Duties include conducting phone assessments; Receiving calls via call center database Network of Care (NOC), Mediware (SAMS) or equivalent; Technical, and informational assistance to individuals and subcontractors; Coordinating services with other agencies; Providing resource information or referrals to elderly clients, their family members, and/or caregivers; Responding to telephone inquiries from seniors, people with disabilities and their caregivers who present multiple and complex problems; Conducting brief assessments over the phone and efficiently screens callers for appropriate services; Coordinating services with internal/external service providers; Providing options counseling; Providing information and referrals resources; Creating and maintaining electronic or equivalent records of clients/providers; Maintains client information in NOC and/or SAMS. Multitask in a call center environment; Creating/maintaining auto and on-going monitoring of clients' status and determining eligibility; Providing information and referral, legal services, crisis and benefits counseling and services necessary to stabilize a situation for elderly clients and family members on a short-term basis; Maintaining confidential records that may include client data; Completing reports as required; Adhering to quality assurance plan, policies and procedures per internal and external agencies; Properly documenting time in ADP or its equivalent. Desired Education/Job Experience: High school graduation or its equivalent; or two (2) years of experience and training which provides the required knowledge, skill, and abilities; any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. Preferred Benefits Counselor I certification within 6 months of hire date This position is a Pay Grade 7, with a starting hourly rate of $15.28. This is a non-exempt position. Open until Filled.
