AARS Mental Health Case Managers support people in our community who are impacted by mental health issues, many of whom are affected by substance use and trauma. We are the first line of support when community members get released from the criminal justice system and emergency psychiatric services. Key Responsibilities
Provides targeted case management services at the clinic/agency site, in the community or in-home to our Transitional Age Youth clients (TAY) from ages 16-25.
Maintains timely impeccable documentation of services in agency’s Electronic Health Record system (Welligent) using Short-Doyle Medi-cal documentation.
Works within a multi-disciplinary team of clinicians, peer mentors, clinical supervisors, psychiatrists, case managers and intake coordinators.
Maintain positive working relationships with outside providers, community-based organizations and our County funders.
Utilize evidence based practices and models.
Adhere to State, Agency, County requirements relating to processes, documentation, productivity and quality improvement.
Provide targeted case management services as appropriate in the community and at the agency site as assigned.
Facilitate and support clients’ movement throughout the program with particular attention to need of ancillary resources and progress toward meeting goals outlined in treatment plans.
Facilitates orientations, coordinates care with client and key stakeholders, assists in providing client education and support clients at external appointments.
Participate in development of discharge plans and levels of care including extension requests based on need.
Attends required trainings and meetings.
Plans and facilitates client celebrations and special events as assigned. May be asked to work on a weekend or during after-hours for crisis stabilization, home visits, community education, resource fairs, outreach and special events.
Properly documents all case management sessions and attempts to contact in a timely manner.
Maintains accurate records by data entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Will participate in Peer Chart/ Utilization Reviews for preparation of program audits. Will adhere to documentation policies and procedures set forth by Compliance Department.
Be responsive and pro-active in all correspondences with referring parties, judges, probation, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc. Will provide informed, written status reports to the above entities as appropriate.
Education and Knowledge, Skills and Abilities
Education Required:
Bachelor’s Degree in Counseling, Social Work, or related field with current registration with a Drug and Alcohol Certification program recognized by DHCS or Mental Health Rehabilitation Specialist (MHRS) credential issued by the County for paraprofessionals who have a bachelor's degree plus four years of experience in a mental health setting. (A master's degree in a mental health related field may substitute for two years of experience. A two year college degree plus six years of experiences would also meet the requirement qualifications.)
First Aid Certified within 30 days of employment
CPR Certified within 30 days of employment
Valid California driver’s license
Desired:
Certification or final stages of certification with a Drug and Alcohol Certification program recognized by DHCS
Background Clearance Required:
Must not be on active parole or probation
Knowledge Required:
Working knowledge of targeted case management services
Knowledge of the criminal justice system
Knowledge of mental health and substance abuse recovery principles and resources; including Co-occurring disorders and trauma informed treatment
Knowledge of cultural stigmas of counseling and treatment services
Knowledge of principles and techniques of social work, case management, crisis intervention, client centered counseling, and group facilitation
Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications
Skills and Abilities Required:
Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data
Professionalism, punctuality, flexibility and reliability are imperative
Excellent verbal, written, and interpersonal skills
Integrity to handle sensitive information in a confidential manner
Excellent organization skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow-through with tasks
Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility
Strong initiative and enthusiasm and willingness to pitch in whenever needed
Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations
Able to work within a frequently changing project scope while maintaining overall direction and structured priorities
Ability to work a flexible work week schedule; including at minimum one late day (until 8pm)
Desired:
Experience working with criminal justice population
Experience working within the Santa Clara County –Department of Behavioral Health Services system
Verbal and written skills in one of the County’s threshold languages, including Spanish, Vietnamese, Tagalog, Mandarin/Cantonese
