Job Summary and Scope
Social Worker Case Manager provides complex care coordination services to patients and their families by managing the medically related social, cultural, and emotional needs of the individual based on their medical condition, treatment, recovery, and transitional care post hospitalization. Ensures and coordinates effective discharge planning and case management services to patients and families to include assessment, planning, implementation, coordination, monitoring, and evaluation and work closely with the physicians, registered nurse case manager, nursing staff, multidisciplinary team, and serve as a community resource liaison.
Essential Functions:
POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs.
PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops. Establish personal networks and participate in professional societies.
Collaborate with patient, families, caregivers, physicians, registered nurse case managers, case manager assistant, nursing staff, community providers, and multi-disciplinary team to identify and coordinate continuum of care needs, including discharge education, assessment, and planning. This includes obtaining necessary data regarding the patient’s socio-economic and psycho-social history as well as providing education and community resources creating an individualized discharge plan.
Make timely and appropriate referrals for post-acute services including home care, durable medical equipment, LTACH, dialysis facilities, acute rehabilitation facilities, specialized post-acute facilities, and skilled nursing facilities to assigned patients. Identify change in patient condition and assist with or institutes appropriate actions in a timely manner.
Promote patient's independence by incorporating and engaging patients and families to participate in individualized discharge preferences and establishing patient care goals, coordinating family meetings, teaching patient, friends, and family to understand condition, medications, and self-care skills; and answer questions. Resolve patient problems and needs by utilizing multidisciplinary team strategies.
Escalates complex cases to director of case management.
Increase patient satisfaction (HCAHP) scores by using AIDET and departmental initiatives.
Complete assigned risk assessments, and discharge summary documentation to provide continuity in communication among multi-disciplinary team. Maintain patient confidence and protect operations by keeping information confidential and medical records complete and accurate. Adheres to standard documentation process by charting in patients electronic medical record and department records.
Assure quality of care by adhering to quality safety goals, policies and procedures, regulatory and legal compliance, therapeutic standards, measuring of health outcomes against patient care goals and standards; make or recommend necessary adjustments.
Compliance in Social Worker standard work duties.
Participate in the hiring, orientation, precepting, evaluation, and/or retention of co-workers.
Perform miscellaneous job-related duties as assigned.

Accountability
Maintain state and federal regulatory standards relating to case management and discharge planning.
Coordinate effective patient through put and enhance timely discharges in accordance with Length of stay (LOS) benchmarks
Supervise clinical social work for MSW licensed personnel in Case management department as assigned. Supervision will include meeting with MSW licensed personnel to review work, complete clinical review documentation, and be available for questions
Communication Skills
Excellent verbal and written communication skills
Required Qualifications
NM MSW license required
Knowledgeable in computer skills, Microsoft Word, Excel, Outlook, Cerner, Epic and Case management systems as Curaspan, Navihealth, Midas , Allscripts, etc.
Must have knowledge of Interqual and Milliman care guidelines.
Knowledge of Medicare & Medicaid regulations
Knowledge of insurance provider and other third party payers.
Must have experience in discharge planning and a working knowledge of alternative delivery systems including home health, durable medical equipment, rehabilitation, extended care facilities, and skilled nursing facilities.
Must be able to multitask and work independently
Education/Experience
Education:
Master’s in Social work from an accredited school
Licenses/Certifications:
Licensed Clinical Social Worker (LCSW) in the state of New Mexico or Licensed Independent Clinical Social Worker (LISW)
Work Experience:
2 years’ experience in related field
Preferred Educational/Experience Requirements:
LCSW or LISW in the state of New Mexico
Case Management Certification (CCM) or American Case management association certification (ACM) preferred or at least 3 years after hire
Supervision
Works under the direction of the Director of Case Management
Conditions of Employment
Must pass a pre-employment criminal background check, reference checks and a post offer drug screen.
Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).
