Relief - Case Manager

sought to join our team in Ventura, in this role you will have general case management duties including intake, assessment, case record management and discharge preparation for our clients.

As needed up to 19 hours per week.

Community Solutions Inc. (CSI) is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved, or at risk of involvement, in the child welfare, juvenile justice, or criminal justice system. Since CSIs founding in 1962, it has been a pioneer in the community-based service. We have been delivering supervision, treatment, and comprehensive individualized services to help clients interact more effectively in their communities and become productive citizens. Using this plan we built the first ever halfway house in New England which has now been replicated and grown to almost 50 programs in multiple states. For over 50 years, CSI has been building upon its successful foundations and has been expanding its services to serve thousands of adults, youth, and families each year.

ESSENTIAL FUNCTIONS include but are not limited to:

Within five (5) days of referral, collect all intake information and enter into agency data collection systems on the day the data is obtained.

Perform client needs assessments within five (5) days of intake and enter data into CSI data systems on the day the data is obtained.

Make referral to groups (as appropriate) within fifteen (15) days from the date of intake and orientation.

Actively participate in Case Planning Team Meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge. Provide and elicit peer support for client issues.

Develop and document program plan goals with clients within five (5) days of intake and review with client on a bi-weekly basis. Monitor treatment stipulations, job readiness preparation and community resource referrals.

Schedule, conduct, and document individual case management sessions that address client personal issues and program performance. Enter each session enter into CSI data systems on the day the data is obtained.

Based on work flow of the office, a CM may be assigned group facilitation responsibilities.

Maintain client files and ensure compliance with agency policy regarding confidential information.

Submit monthly progress reports for clients to your supervisor four (4) days before due date.

Complete incident reports as needed and submit to supervisor prior to leaving shift.

Tape all sessions with clients and follow procedure to upload these recordings for quality assurance review.

Actively participate in quality assurance review session and apply new strategies and techniques presented.

Register for and attend all required trainings and boosters including but not limited to Motivational Interviewing, Case Management and Assessment.

Provide coverage of front desk and perform the associated duties.

Participate in the administration and collection of WAI surveys.

Secure urines from same sex clients and complete all required paper work and documentation including agency data collection systems on the day the data is obtained.

Monitor case load to ensure all urines and breathalyzers are requested and are random.

Serve as a role model to clients and staff modeling pro-social behaviors.

QUALIFICATIONS:

Bachelor’s degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.

Excellent oral, written and interpersonal skills.

Ability to engage and develop collaborations with community agencies.

Previous assessment experience preferred.

Experience with Microsoft Office
