Responsible for leading and implementing Care Signature initiatives across Yale New Haven Health System (YNHHS). Creates a collaborative culture amongst clinical experts and stakeholders, and uses clinical expertise and experience to engage clinicians. Focuses on opportunities to reduce variability, facilitate clinical consensus, improve efficiency in provision of care, and enhance quality and safety. Relies on a data and evidence driven process to improve outcomes, equity, and access across YNHHS.

1. Utilizes clinical experience and expertise, in collaboration with groups of clinical experts and stakeholders, to facilitate the founding principles of Care Signature.
2. Works with stakeholders in Care Signature Councils to define clinical best practice, and develops YNHHS consensus statements for areas where evidence is inconclusive.
3. Identifies opportunities for process improvement or clinical redesign.
4. Coordinates communication and engagement of a multidisciplinary team related to Care Signature work.
5. Represents and shares Care Signature work with the Care Signature team and leadership group.
6. Inspires others to live our mission by regularly and actively soliciting new Care Signature ideas. Works with clinical leadership to explore and bring forth a well-executed product.
7. Develops appropriate metrics and employs analytical tools to track performance, and iterates on results.
8. Identifies and regularly messages how initiatives enhance the value equation by influencing quality, safety, equity, and patient/provider experience. Maintains a high level of engagement throughout.
9. Provides high quality, well thought out work products. Seeks feedback on work product from colleagues.
10. Collects and analyzes data to support value-added improvements in clinical outcomes, operational efficiencies and/or improved financial performance.
11. Ensures efficient and timely response to assigned deliverables. Identifies risks or barriers to accomplishing specific tasks or broader goals.
12. Identifies appropriate clinical stakeholders and other needed resources (Epic, JDAT, other departments) to ensure complete and effective team membership.
13. Effectively scopes projects to achieve outcomes within a defined timeline.
14. Appropriately escalates barriers with project leader, sponsor, physician partner, and other key stakeholders.
15. Conducts research and reviews literature on relevant topics prior to the launch of the project. Synthesizes and presents possible consensus opportunities for YNHHS.
16. Contributes to presentation of team?s work at local, regional, or national conferences and to publication in peer-reviewed journals.
17. Manages projects of increasing scope and complexity with success.
18. Disseminates tools, teaches our approach and supports work in new ways.
Qualifications
EDUCATION
Bachelors Degree in healthcare or related field is required. Masters Degree (or equivalent certification) in clinical healthcare (PA, APRN, PharmD, etc) is strongly preferred.
EXPERIENCE
Minimum of 5 years professional work experience, with experience in healthcare, project management, or quality improvement preferred. Experience in data analysis, reporting and effective presentation preferred. Proficient in the use of the Microsoft Office Suite, including Excel and PowerPoint.
LICENSURE
Certification and licensure to practice clinically in professional degree field at YNHHS, as applicable.

Demonstrated ability to organize resources and establish priorities. Excellent verbal and written communication skills. PHYSICAL DEMAND
Must have the ability to travel to various sites across the health system.
