At SeniorBridge, we are passionate about helping families and loved ones find solutions that make life better. We provide concierge care management, private duty nursing, and personal care assistance services to seniors.

Responsibilities

As an RN Home Care Manager, you will provide and coordinate care to clients and their families using a multidisciplinary approach.

Conduct home care visits and telephonic care management for Private Pay clients.

Develop a professional care plan for home health aides from the assessment findings and in collaboration with the client's physician.

Ensure in-home care delivery is in accordance with each client's contractual program requirements, is high quality, and complies with company-adopted evidence-based clinical practice guidelines.

Create thorough and timely documentation of initial and ongoing assessments and outcomes

Clearly communicate the care plan and the subsequent cost of all services to client and/or families, attorney, guardians, etc.

Act as a liaison to families as well as with collateral clinicians, professionals and agencies.

Work effectively with all departments to ensure the best delivery of service

Participate in on-call activities as directed by supervisor

What you will need for this role:

Experience in Home Care

Strong clinical skills and the ability to make independent decisions in the field

Excellent communication skills and a team approach to care

Knowledge of community resources for seniors

Infusion skills are a plus

Required Qualifications

Licensed Registered Nurse in the state of New York with no disciplinary action

2 years of experience of in home case/care management

Experience working with the adult population

Knowledge of community health and social service agencies and additional community resources

Ability to travel to client residences

Exceptional communication and interpersonal skills with the ability to quickly build rapport

Ability to work with minimal supervision within the role and scope

Ability to use electronic information applications/software programs including electronic medical records

Intermediate computer skills and experience with Microsoft Office products

Excellent keyboard and web navigation skills

Ability to work a full-time (40 hours minimum) flexible work schedule

This role requires and individual to have a valid state driver's license, a clean driving record and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits.

This role is considered patient facing and if selected, you will be required to be screened for TB.

We will require full Covid vaccination for this position.
