The Social Worker will identify and assess available services to meet the social and emotional needs that affect a patient's medical condition, treatment, and recovery.
Responsibilities
Essential Duties and Responsibilities
Assures that patients and families are knowledgeable of available community resources and how to access them.
Serves as a resource to clinical staff in dealing with complex psychosocial issues and accessing community resources.
When appropriate, provides direct Care Coordination services to patients with complex social/emotional needs.
Documents patient/family status, diagnosis, treatment plan, goals, and interventions, evaluation results, observations and progress in medical record.
Provides assistance with transportation services for medical visits.
Provides Food Bank referrals, Meals on Wheels referrals and other primary resources as needs are identified.
Provides education on supplemental services for assistance, eye care and DME.
Acts as resource for materials on disease states, diet, general health, stress management.
Provides Legal service referrals.
Provides assistance to obtain sitters and agencies for assistance at home.
Serves as a Community resource expert for local services and support groups.
Provides support for Case Management and the clinical team as needed to coordinate services listed above.
Refer complex patient concerns to the member's physician and medical director for appropriate interventions.
Works closely with the hospital case managers, attendings, PCP and medical director, community agencies and other approved vendors to ensure the member's healthcare needs are met.
Acts as resource and is able to facilitate eligibility and benefit information
Acts as a resource and is familiar with the provider network
Maintains accurate and concise documentation and adheres to documentation guidelines
Adheres to policies and procedures
Maintains confidentiality and adheres to HIPPA guidelines at all times
Please Note: This position will be 80%-90% Work From Home, however, it does require travel to different Conviva Clinics in the Northern part of Palm Beach Country to meet with our patients
Requirements:
A current, unrestricted social work license in your state of residence
Candidate MUST have a minimum of 2 years of post-graduate social work experience (No New Grads)
Minimum Education: Bachelor's Degree in Social Work; Masters in Social Work Preferred
Exerpeince in Acute Care (Adult/Geriatric Population)
Excellent Customer Service
Must interact with patients in a respectful and amicable manner.
Employees are expected to treat patients with compassion and protect their rights.
Adhere to all Company policies.
Skills/Minimum experience:
Case Management experience
Expert knowledge of community programs/services
Experience in discharge planning
Experience working in managed care
Excellent written and verbal communication skills
Computer Proficiency - Internet, Microsoft Word, Microsoft Excel, Email
Bilingual in English and Spanish preferred (not a requiremnet)

Interview Format: MODERN HIRE
As part of our hiring process, we will be using an exciting interviewing technology provided by Modern Hire, a third-party vendor. This technology provides our team of recruiters and hiring managers an enhanced method for decision-making.
If you are selected to move forward from your application prescreen, you will receive correspondence inviting you to participate in a pre-recorded Voice Interview and/or an SMS Text Messaging interview. If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone or computer to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
Please note:
Associates are required to be fully COVID vaccinated, including booster, or undergo weekly COVID testing and wear a face covering while at work. The weekly testing will need to be done through an approved Humana vendor, and unvaccinated associates should follow all social distancing and masking protocols if they are required to come into a Humana facility or work outside of their home.
