Profile

The CHHA Liaison Coordinator assesses, plans, and coordinates home care plans for clients in accord with professional standards, regulatory guidelines, and Agency policies.  This position also provides consultation, education, and interpretation of programs and services to the general public and referral sources.

 

       

*       Current clinical knowledge regarding pathophysiology, current disease treatment modalities and drug therapy parameters *       Knowledge of clinical documentation protocols, relevant home health care regulations and standards, and home health care fiscal requirements *       Ability to communicate successfully both orally and in writing including effective listening skills *        High level of interpersonal skills and ability to work effectively with others *        Ability to plan and organize work schedule to include effectively setting priorities where necessary *  May be required to train or assist in training other staff *  Instructs and evaluates ancillary personnel on their role in the plan of care

Requirements:

*License and current registration to practice as a Registered Nurse in New York State *Baccalaureate Degree in Nursing from an approved program preferred *Two (2) years recent medical surgical experience in a hospital setting *Ability to climb stairs and ambulate at the various sites *Must have a valid driver's license and ability to provide own transportation during working hours *Equivalent combination of education and experience
