Description
The Staff Consultative Pharmacist consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy. The Staff Consultative Pharmacist work assignments involve moderately complex to complex issues where the analysis of situations or data requires an in-depth evaluation of variable factors.

Summary:
To practice as a palliative care clinician for hospice patients, provide end-of-life pain and symptom management and improve patient's quality of life.
Essential Duties and Responsibilities:
Clinical Consultations
Recommends appropriate pain and symptom management therapies based on patient specific information
Provides real-time collaboration with nurses to create a cost-effective medication care plan
Focuses on medication misadventure prevention by providing education to nurses and caregivers
Enhancing patient's quality of life
Documentation
Advances patient care by documenting interventions for outcomes measures
Maintains accurate patient profiles
Verifies encounter information is complete before submitting to be transcribed/processed

This position is for Enclara Pharmacia, a Humana company. Enclara, is a wholly owned hospice and palliative care pharmacy business acquired by Humana in 2021. Enclara is the nation's largest full-service pharmacy benefits manager (PBM) specifically for the hospice and palliative care sectors. Serving over 450 customers and nearly 100,000 patients nationally, Enclara works closely with hospices to reduce pharmacy costs, improve patient care and support caregivers through: digital innovations, flexible medication access, one-on-one clinical support, and excellent customer service.

Education and/or Experience:
PharmD from an accredited school of pharmacy or a BS with significant relevant experience with hospice care or oncology
