The Community Health Worker contributes to the administration of Care Management and Utilization Management. Community Health Worker collaborate with Case Managers (CMs) and Utilization Management (UM) Nurses to address co-occurring physical health, behavioral health, and social factors affecting members. The individual improve members' understanding of and ability to self-manage their chronic conditions and Social Determinants of Health (SDoH) and work to reduce disparities in outcomes associated with chronic conditions and SDOH.

Responsibilities

This is a field-based position requiring approximately 90% travel to members' homes/provider offices, medical facilities, etc. within the designated area. Candidates must reside within a 25 mile radius of one of the following locations, Orlando, Miami, or Tampa Florida to be considered for this position.

Position Responsibilities

The Community Health Worker builds visibility and credibility of Humana's products and services throughout the community via chats, posts, and interactions. The individual in this role engages new community members and strategizes methods to scale communities and drive connections among all community members. The Community Health Worker understands professional concepts and standards, regulations, strategies, and operating standards. This individual makes decisions about their own work approach and priorities and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.

Conduct in-person non-clinical assessments to understand enrollee care needs, preferences, and socioeconomic barriers, and evaluate the home environment as needed

Locate enrollees who are unable to contact to assess for Social Determinants of Health, identify barriers and provide resources as applicable.

Work collaboratively with other Humana associates as a member of Humana's Care Management Support t

Assist enrollees with scheduling physical health and behavioral health office visits and addressing barriers to appointment attendance, barriers to healthy living and accessing healthcare services and arrange necessary social services and supports.

Act as an enrollee advocate with providers, community resources, schools, and others, including accompanying enrollees to provider office visits as requested.

Must be passionate about contributing to continuously improving consumer experiences.

Required Qualifications

Experience working with people with SDoH challenges, community health agencies/social service agencies (Area Agency on Aging, DME providers, Meals on Wheels etc.)

Knowledge of and ability to navigate community resources

Become a Certified Community Health Worker (CHW) within 18-months from hire date

Proficiency in Microsoft Office applications (Word, Excel, Outlook)

Ability to manage multiple priorities in a fast-paced environment

For the Miami location Must be Bilingual Spanish/English with the ability to read, write and communicate effectively in both languages and be willing to take a test

Preferred Qualifications

Experience with case management, discharge planning, crisis intervention and patient education for adult acute care

Bi-Lingual (English/Spanish)

Medicaid Program experience

Experience working with members/patients with medical and mental health co-morbidities

Monday through Friday 8:00 -5:00 PM Eastern Time, hours may be flexible based on business needs

Travel:

This position requires about 90% local travel to meet with members, providers and facilities

Work at Home Guidance

To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria:

At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested

Satellite, cellular and microwave connection can be used only if approved by leadership

Associates who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.

Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job.

Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

Driver's License, Automobile, Insurance Policy

This role is a part of Humana's Driver Safety program and therefore requires an individual to have a valid state driver's license, reliable transportation, and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits

TB Screening

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB

Agency For Health Care Administration

Bilingual English/Spanish Proficiency Required for Miami Location

Bilingual Language in both English and Spanish - Language Proficiency Assessment will be performed to test fluency in reading, writing and speaking in both languages.
