The Referral Coordinator gathers and communicates all relative information and preparation instructions to patient and referring providers. Decisions typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.

CenterWell Senior Primary Care has a strong emphasis on senior-focused primary care for members of Medicare Advantage health plans and is committed to providing personalized, high-quality primary care combined with an excellent patient experience. CenterWell Senior Primary Care has experience in both the treatment and management of most chronic and acute-care conditions. The practices also provide health education and value-added, well-being services at the centers and around their neighborhoods to help both patients and community members improve their health.
The Referral Coordinator will be responsible for daily activities to include:
Gathers and communicated all relative information and preparation instructions to patient and referring providers
Decisions regarding interpretation of area / department policy and methods for completing assignments
Prepares and processes all managed care referral paperwork
Contacts various offices / facilities to obtain necessary appointments for completing referrals
Maintains referral records and files documentation
Responds to patient inquiries regarding health plan referrals
Maintains strictest confidentiality with adherence to all HIPPA guidelines / regulations
Vaccine Policy:
For this job, associates are required to be fully COVID vaccinated, including booster.

Provide documentation for a medical or religious exemption consideration.

Required Qualifications
1 - 3 years of hands-on healthcare experience in a clinical setting, preferably within the Medicare population
Previous medical referral experience
Proficient with medical terminology
Proficient with Microsoft Office applications including Microsoft Word, Excel and Outlook
Experience with Electronic Medical Records
Excellent professional communication abilities, at all levels within the organization and with patients, at all times
Adhere to Humana's current COVID vaccination policy
This role is considered patient facing and is part of the company's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB
Must be able to work at the clinic located at: 3151 South Second Street, Louisville, KY 40208
Preferred Qualifications
Bachelor's Degree in Healthcare or Business Administration
Knowledge of ICD 10 and CPT coding
Medical Assistant or Professional State Certifications related to healthcare
Bilingual in English and Spanish
Referral Coordinator Working hours:
Scheduled 40 hours per week
Monday to Friday 8AM-5PM
Local travel may be required; Mileage is reimbursed

Language Proficiency Testing:
Bilingual English/Spanish. Must be able to speak, read and write in both languages without limitations or assistance. See Additional Information on testing. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.
