Special Qualifications
: Per PA Dept. of Welfare regulations must be a self-identified user of mental health or co-occurring services who is trained and certified to offer support and assistance in helping others in their recovery. If not currently certified, certification must be successfully completed upon hire before providing services.

ESSENTIAL FUNCTIONS:
1. Provide peer support services to an assigned caseload utilizing a strength-based model of services:
Support individuals in identifying their psychiatric, social, emotional, and economic needs
Work cooperatively with individual, involved family members, behavioral health team members, and supervisor to ensure all needs are being met
Focus on strengths, interests, and personal goals during assessment and throughout service
Assist individuals in developing personal goals for recovery and wellness
Work collaboratively with the individual to reach established personal goals
Teach and role model the value of every individual’s recovery experience
Reinforce that recovery is not only possible but probable
Promote individual’s use of self-directed recovery tools
Teach and role model appropriate coping techniques and self-help strategies.
Advocate for individuals in securing necessary services and resources to meet their needs
Assist individuals and families managing personal crisis
Promote and provide guidance with the WRAP process
Assist in the identification and development of opportunities for community integration
Provide outreach to individuals who miss scheduled appointments, appear to need extra support or have been discharged from other programs
Communicate effectively regarding individual progress through reports, records and team meetings.
2. Complete necessary documentation required by state regulations and Home Nursing Agency.
Ensure all required documentation is accurate, legible, and in the allowable time frame
Use person-centered, recovery-oriented language in all documentation
Collaborate with the individual on daily service documentation, goal plans, and goal plan reviews.
3. Provide peer support services which will assist individuals in attaining the following outcomes:
Decrease in number of inpatient stays
Minimum contacts with Crisis Center
Entrance into desired vocational or educational services
Promote recovery oriented skills and tasks

4. Attend weekly clinical and staff meetings as scheduled.
5. Participate in educational programs, quality assurance, and quality improvement activities.
6. Maintain and protect individual and Agency confidentiality.
7. Promote personal and co-worker safety during work duties.
8. Represent and participate in Agency professional and community activities as requested.
9. Adhere to Agency and professionally acceptable standards, policies, and procedures.
10. Work to achieve and maintain quality and customer satisfaction in the delivery of services to internal/external customers.
11. Demonstrate support of Agency mission and philosophy in the delivery of services.
12. Perform other duties and responsibilities as requested
