JOB SUMMARY
This position will support Carilion's hallmarks of service excellence and quality for all job responsibilities. Will assist the RN in performing care for patients of all ages in the home specific to the individual patient needs as ordered by the physician and supervised by the registered nurse. Care must be directed toward specific goals and documentation must reflect care given as well as discharge planning.
Provides quality nursing care to meet the physical, emotional, and psychosocial needs of the patient of all ages based on nursing processes. The Home Health LPN may not perform initial assessments for home health admission nor home health aide evaluations. The LPN is supervised by the RN case manager, Clinical Supervisor, or Manager. The individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. As such, the individual must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess date reflective of the patient's status, interpret the appropriate information needed to identify each patient's requirements relative to his/her age and specific needs, and provide the care needed as described in departmental policies and procedures. Largest volume of time is spent while sitting during activities such as driving, patient care and completing documentation. May be required to lift/transfer or turn patients weighing in excess of 200+lbs. May need to carry equipment weighing in excess of 50lbs. Frequent exposure to temperature extremes depending upon individual home environment as well as seasonal weather conditions. Frequent travel in adverse weather conditions such as rain, snow, sleet, humidity, etc. Required to operate motor vehicle daily. There is potential for exposure to elements such as chemicals in blood collection tubes. Potential exposure to blood/body fluids. Contact with fumes, odors, dust, smoke, etc. may be encountered in home environments. May be able to work independently with minimal on-site supervision. Must be able to effectively interact with other members of the health care team. Shift work occurs with evening and weekend visits. Patient homes often limited working space. Stressful documentation requirements. May have visits in high crime areas.

MINIMUM QUALIFICATIONS
EDUCATION: Graduate of approved school of Practical/Vocational Nursing.
EXPERIENCE: Must have at least one (1) year of experience in med-surg nursing. Home health experience preferred.
LICENSURE, CERTIFICATION, AND/OR REGISTRATION: Licensed Practical Nurse in the Commonwealth of Virginia. Must possess valid Virginia driver's license.
OTHER MINIMUM QUALIFICATIONS: Must have own form of reliable transportation and clear criminal record check.
