Description
The Field Care Manager 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Field Care Manager 2 employs a variety of strategies, approaches, and techniques to manage health and well-being concerns of Medicaid enrollees from an integrated Medical and Behavioral Health approach with added focus on addressing social determinants of health.
An associate in this role Identifies and resolves barriers that hinder effective care while using the support and resources of the Humana Medicaid Interdisciplinary Care Team and the enrollee's multi-disciplinary team.
The Field Care Manager 2, through regular face to face and telephonic contact, active assessment and individualized care planning proactively assists in helping enrollees meet health, well-being and social determinant of health goals.
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Required Qualifications
Registered Nurse
Possess 2 years of experience of in home case/care management
Experience working with Medicaid enrollees to include TANF, and enrollees with special health care needs.
Ability to travel to member's residence within 50 mile radius
Exceptional communication and interpersonal skills with the ability to quickly build rapport
Ability to work with minimal supervision within the role and scope
Ability to use a variety of electronic information applications/software programs including electronic medical records
Intermediate to Advanced computer skills and experience with Microsoft Word, Outlook, and Excel
Excellent keyboard and web navigation skills
Additional Requirements
Position is located in Lexington or Western Kentucky
This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB
Hours: Ability to work a full-time (40 hours minimum) flexible work schedule (Monday - Friday 8:00 AM-6:00 PM)
This role may require limited travel in Lexington or Western KY, to support business needs.
This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
Work at Home Requirements
WAH Requirements - Must have accessibility to high speed DSL or Cable modem for a home office (Satellite internet service is NOT allowed for this role); recommended speed is 10Mx1M
Preferred Qualifications
Master's degree in Nursing and mental / behavioral health experience
