Description

SeniorBridge is looking for a PRN Field Care Manager for at least 15 hours per week midweek and participation in the On-Call rotation every 3-4 weeks.

Responsibilities

As RN Care Manager, you will direct care to clients and their families using a multidisciplinary approach. This position includes the following:

Conducting admissions and assessments to identify problems and necessary services

Developing a professional care plan from the assessment findings and in collaboration with the clients Physician

Ensuring in-home care delivery is in accordance with each client's contractual program requirements and is high quality and complies with company-adopted, evidence- based clinical practice guidelines

Thorough and timely documentation of initial and ongoing assessments and outcomes

Communicate explanation of the care plan and all subsequent cost of all services to client and or Families, Attorney or Guardians

Act as a liaison to families as well as with collateral clinicians, professionals & agencies.

Work effectively with all departments to ensure the best delivery of service

Participate in case conferences with other disciplines providing care

Participate in the on call activities as directed by supervisor

Observes infection control practices and uses standard precautions

Participates in quality management/performance improvement activities

Schedule: Must be available at least 15 hours per week midweek and participate in the on-call rotation every 3-4 weeks. On-Call is Monday-Sunday. Coverage is 5PM-8:30AM weekdays and 24 hours on weekends.

Required Qualifications

Registered Nurse with FL state licensure, no restrictions

1 year of acute care experience of in home care management

Experience working with a geriatric population

Knowledge of community health and social services and additional community resources

Ability to travel to member's residence within 30 to 40 miles

Exceptional communication and interpersonal skills with the ability to quickly build rapport

Ability to work with minimal supervision within the role and scope

Ability to use a variety of electronic information applications/software programs including electronic medical records

Associates working in the State of Florida will need ACHA Level II Background clearance

This job requires close personal interaction with patients. You will not be able to social distance.

If progressed to offer, you will be required to provide proof of full vaccination where allowed by law. Medical and religious exemptions will be considered on a case by case basis.

This role is a part of Humana's Driver Safety program and therefore requires and individual to have a valid state driver's license, a clean driving record and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limit.

This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

3-5 years of in home assessment and care coordination experience

Bilingual - English, Spanish
