The Social Worker provides social work interventions for Medical Center patient and family needs. Some responsibilities include but are not limited to: coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies; performs a psycho/social assessment for continuity of care planning for in/outpatients; provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability. Master's of Social Work from an accredited college or university

Qualified to receive Registration as an Agency Affiliated Counselor by the State of Washington Department of Health required

Strong interpersonal skills; a working knowledge of individual and family dynamics; strong organizational skills, an understanding of systems and how to work within systems; an understanding of the impact and implications of disease and the course of disease; excellent communication skills; the ability to perform psycho/social assessments; and a working knowledge of community resources.

Preferred: Licensed Advanced Social Work (LASW) or Licensed Independent Clinical Social Work (LICSW)
