Job Description
This Clinical Case Manager position is 100% remote and candidates can apply from any state but must be able to work EST hours.
The Clinical Case manager is a licensed Clinical Social Worker position.
Normal Working Hours: Monday through Friday 8:00am-5:00pm EST with flexibility to work outside of these standard hours per the business need.
No evenings, weekends, holidays, or travel will be required.
The Clinical Case Manager partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs.
The Clinical Case Manager will work with a population spans the entire lifecycle.
All member contact is done telephonically or via electronic exchange.
The Clinical Case Manager's role is to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
Pay Range
The typical pay range for this role is:
Minimum: 45,240
Maximum: 98,280
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications
- Candidate is required to have the following licensure: Licensed Master Social Worker (LMSW),
- Must have an active current and unrestricted state license in state of residence
- Must be willing and able to obtain multi-state licensure (minimum of 10 states) before new hire training is completed (additional licensures will be paid for by the company).
- 2 years' experience in medical social work or case management
Preferred Qualifications
- Managed Care experience
- Telephonic Social Worker experience

Master's Degree required
