Responsibilities

Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership.

Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems.

Recommend formulary alternatives when appropriate.

Compound and dispense medications for accurate delivery to patients.

Provide drug information and education to medical staff and patients.

Communicate with physicians and other clinicians as necessary to facilitate care.

Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.

Assist with academic functions such as serving as a preceptor for pharmacy students.

May float to provide coverage of pharmacy needs.

Provide patient counseling when required.

Requirements

Licensure / Certification / Registration:

Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education:

Bachelor's degree in Pharmacy or Doctorate of Pharmacy required.

Pharmacy residency preferred.

Additional Preferences

Work under a Collaborative Drug Therapy Management Agreement (CDTM), which allows qualified pharmacists within the context of a defined protocol to perform specific patient care functions such as modifying pharmacologic drug therapy regimens; dose, frequency, discontinuation and/or initiation, renewal of existing prescriptions.

Under the Pharmacotherapy Clinic Scope of Services policy, the clinical pharmacist(s) may partake in the following, in consultation with the patient’s PCP, specialist provider, and/or the medical director:

o Obtain patient past medical history and assess the chronic/specialty disease state(s) as indicated on the patient referral form including, but not limited to: anticoagulation, asthma, CHF, COPD, diabetes, hyperlipidemia, hypertension, medication assistance, thyroid disorder, tobacco cessation, and other chronic/specialty disease states or acute conditions diagnosed by the patient’s PCP, referring, or specialist provider.

o Obtain patient medication history (including prescription, over-the-counter medications, and dietary supplements)

o Identify medication non-adherence and address contributing factors

o Evaluate and modify pharmacologic drug therapy regimens including initiation, discontinuation, and/or adjustment of drug dose, and renewal of existing prescriptions (including durable medical equipment such as glucometers, test strips, blood pressure cuffs, etc.) as clinically appropriate

Clinical pharmacists under this policy will not initiate or modify any controlled substances prescriptions

o Perform drug regimen review to assure the use of the most cost-effective medications through use of therapeutic alternatives to help improve patient compliance, and to avoid duplicative or unnecessary therapy

o Order and evaluate disease state specific laboratory tests

o Educate patients on disease states, medication therapy, medication adherence, and use of durable medical equipment

o Screen for necessity of blood glucose and/or blood pressure monitoring supplies

o Document patient visits and incorporate that documentation into the electronic medical record (EMR)

o Consult and coordinate with the patient’s provider as necessary (or with the local AMITA Health Emergency Department if deemed necessary for immediate patient assessment)

o Triage the patient and consult with the patient’s primary care, specialist, or referring provider when a patient presents with a new complaint for which the patient has not been evaluated previously by his or her physician.

o Schedule individualized appointments at a minimum of every 3-6 months with the Pharmacotherapy Clinic to achieve therapeutic goal(s)

Manage chronic disease states, medication adherence, and a provider’s refill approval que by assessing, evaluating, and adjusting medication therapy regimens via telehealth technology.

Supervise certified pharmacy technicians completing direct medication adherence outreach and following specific protocols for prescription refill approvals

Collaborates with others to resolve issues related to patient care, communication, policies, and resources. Create, review, and modify evidence-based pharmacy policies, protocols, procedures and guidelines specific to medication management in order to achieve outcomes for indicators that are pharmacist sensitive.

Qualifications and educational requirements of clinical pharmacists practicing under this protocol include:

o Licensure:

Active licensure in good standing with the Illinois Department of Financial and Professional Regulation (IDFPR)

o Training:

Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 Residency and ≥ 1 years of experience in ambulatory care practice OR Completion of an ASHP accredited PGY-2 Residency in one of the following specialties: Ambulatory Care, Pharmacotherapy, Family Medicine, Cardiology, Internal Medicine, Geriatric, Nephrology, or Transitions of Care OR Doctorate of Pharmacy (PharmD) or Bachelor of Pharmacy (B.S. Pharm) with at least five years of ambulatory care experience in chronic disease management

o Certification:

Clinical pharmacists are preferred to hold nationally recognized certifications (e.g. BCACP, BCPS, BC-ADM, CDCES)
