Responsibilities:

Assists in staffing the departure lounge and overseeing all aspects while patients are in the DL waiting for a ride.

Develops and maintains strong working relationships with external and internal contacts

Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.

Works closely with St. Vincent Outpatient Pharmacy to ensure patient's home medications are delivered and all questions/concerns are resolved prior to leaving.

Utilizes hospital tracking system to facilitate appropriate patients are brought to the departure lounge in order to assure hospital throughput.

Helps ensure a smooth transition for patients discharging via the Ascension wheelchair van.

Responsibilities

Serve as a professional resource to patients, physicians, and patient families as well as community organizations/providers and schools to address issues and concerns relating to patient care services.

Responsibilities:

Establishe contact and rapport with patients and their families, and provide periodic updates to family members.

Intermediate between the patient, family members, physicians, community organizations, schools and care providers, as needed to facilitate communication and resolve issues.

Establishe follow up services, ensuring that all items on the patient care plan have been completed; collaborate with the next level of care including arranging for appointments as indicated.

Address customer service issues with proper follow-up or referral, use service recovery resources, and communicate issues and resolution to appropriate level.

Requirements

Licensure / Certification / Registration:

BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
