*Coordinates, develops, monitors outcomes and implements the Carilion Clinic Sepsis Program. *This position collaborates with other members of the healthcare delivery team, including physicians, to deliver quality healthcare and an evidenced based program. *Facilitates process improvements in employee education, professional practice, systems (EHR), community awareness, patient education and overall program elements. *Drives exceptional patient outcomes *Maintains knowledge in field and is resource to providers and clinicians enterprise wide *Develops system wide metric dashboard and routinely communicates outcomes to Senior and Executive Leaders *Active participant in patient care for identified sepsis admissions (IDRs, rapid responses, patient care and patient planning of care) *Consults, evaluates and prescribes care for septic patient in collaboration with Hospitalist Medical Director (ACP)
Work is accomplished without considerable direction. Works on complex issues where analysis of situations and/or data requires in-depth evaluation of variable factors. Exercises judgment in selecting method, techniques and evaluation criteria for obtaining results consistent with broadly defined policies and practices. Frequently interacts one-on-one, makes group presentations, leads teams, multi-service and organization projects. Frequently must adapt and respond to organization needs, current responsibilities and changing priorities.

MINIMUM QUALIFICATIONS
EDUCATION: Graduate of an accredited School of Nursing required. Bachelor's degree Preferred.
EXPERIENCE: 3 years of experience in program management and in the care of people with chronic disease, sepsis, or critical care required.
LICENSURE, CERTIFICATION, AND/OR REGISTRATION: Current VA RN license required. Must successfully complete orientation and competency requirement.
Must obtain and maintain appropriate certifications as defined by the department.
OTHER MINIMUM QUALIFICATIONS: Demonstrates excellent problem solving, interpersonal, communication, priority setting and work competency skills. Promotes a respectful, inclusive culture of service excellence. Excellent problem-solving skills and ability to exercise independent judgment. Business acumen and leadership skills. Strong verbal and written communication skills with ability to effectively interact with all levels of management, internal departments and external agencies. Working knowledge of various computer software applications. Must be detailed oriented, able to synthesize complex data and utilizes sound clinical judgement based on years of leadership and clinical experience. Savvy with crucial conversations. Intuitive and autonomous, anticipates nursing practice needs.
