JOB SUMMARY
Carilion Clinic is looking for an experienced licensed behavioral health provider (BHP) to join our growing family medicine team. The BHP works as a core member of a multi-disciplinary integrated care team in an academic family medicine practice. Travel between two practices within the Family Medicine Residency is expected and occasional off-site meetings are expected. The role of the BHP is to assist the primary care provider and team improve effectiveness and efficiency in managing the whole patient. The team, led by the primary care provider, includes clinical staff, administrative staff, pharmacists, social work, community health workers, and nutritionists, and other team members.
The BHP strives to be as accessible as possible during business hours, typically seeing patients on-demand throughout the day. The BHP provides a high volume of brief consultation-based interventions to individuals and families without restrictions in gender, age, complexity or medical comorbidity. The BHP also utilizes an approach to assessment and intervention that meets the patient's biopsychosocialspiritual needs and aligns with the Primary Care Behavioral Health model of care. In doing so, the BHP:
*serves as a patient advocate and brokers access to resources outside of the primary care network as appropriate; *educates patients and families; *engages in constant communication with other care team providers to ensure clear expectations and outcomes; *uses approved tools and metrics to monitor patient progress; and *generates billable fee codes when appropriate.
This requires flexibility and adaptability and a willingness to engage in reflective practice as well as provide and receive feedback with the BH and medical team as we learn together and from one another. Lastly, exceptional interpersonal communication and professionalism is paramount as the BHP will interact with learners and providers from multiple health professions at all levels of training and career stages.
MINIMUM QUALIFICATIONS
EDUCATION: Masters in Social Work, Counseling or related field required.
EXPERIENCE:. One to three years of related experience required.
LICENSURE, CERTIFICATION, AND/OR REGISTRATION:Licensed Clinical Social Worker (LCSW)
OTHER MINIMUM QUALIFICATIONS: Effective interpersonal, communication and teamwork skills required. Must successfully complete orientation and competency validation for position.
