Description
The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Responsibilities
The Medicaid Field Care Manager Behavioral Health employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues by advocating, empowering and assisting the member in transforming their current state.
The Medicaid Field Care Manager Behavioral Health identifies and resolves barriers and social determinants that hinder effective care and health outcomes.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
The Role may include creating member's plan of care as well as influencing department strategy, decision making on cases and approaches for project components.
The Medicaid Field Care Manager Behavioral Health will exercise considerable latitude in determining objectives and approaches to assignments.
Knowledge of community health and social service agencies and additional community resources is a plus.
Required Qualifications
The Medicaid Field Care Manager, Behavioral Health 2 must meet one (1) of the following:
Masters Social Work or MS in social sciences, social work, human services or a related field OR
Licensed Mental Health Counselor (LMHC) OR
Licensed Social Worker (LSW) OR
Licensed Marriage and Family Therapist (LMFT) OR
Licensed Clinical Social Worker (LCSW)
Qualified applicants must also meet one (1) of the following:
CAP(Certified Addiction Professional) OR
MCAP(Master's Level Certified Addiction Professional)
The Medicaid FCM, BH 2 must meet all of the following requirements:
1 years Inpatient OR Outpatient Addiction Treatment Experience
3 years of Human Services experience
Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
Must be passionate about contributing to an organization focused on continuously improving consumer experiences
Must be available to work Schedule: Monday - Friday; 8:30 AM - 5:00 PM

Additional Requirements
Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work.
Must have the ability to provide a high-speed DSL or cable modem for a home office (Satellite and Wireless Internet service is NOT allowed for this role). A minimum standard speed for optimal performance of 10x1 (10mbs download x 1mbs upload) is required.
75 - 90% travel in the field to visit members (see Additional information below)
This role is a part of Humana's Driver Safety program and therefore requires and individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits.
This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Preferred Qualifications
1 year Case Management
Medicare/Medicaid Program experience
Experience with case management, discharge planning, crisis intervention and patient education for adult acute care
Experience working with Child, Adolescent and Adult
Experience working with Chronic homelessness, Social determinants of health
Experience working with members/patients with medical and mental health co-morbidities
Managed care experience
Certified Case Manager (CCM)
Bilingual English/Spanish

Interview Process
As part of our hiring process, we will be using an exciting interviewing technology provided by Modern Hire, a third-party vendor. This technology provides our team of recruiters and hiring managers an enhanced method for decision-making.
If you are selected to move forward from your application prescreen, you will receive correspondence inviting you to participate in a pre-recorded Voice Interview and/or an SMS Text Messaging interview. If participating in a pre-recorded interview, you will respond to a set of interview questions via your phone. You should anticipate this interview to take approximately 10-15 minutes.
If participating in a SMS Text interview, you will be asked a series of questions to which you will be using your cell phone to answer the questions provided. Expect this type of interview to last anywhere from 5-10 minutes. Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
