We have an opportunity to join the Alliance as a Care Coordinator I in the Quality and Health Programs Department. There are two positions available and they can be filled in any of our office locations: Merced, Salinas or Scotts Valley.

WHAT YOU'LL BE RESPONSIBLE FOR

In this role, you will:

Assist assigned team with the coordination of health care activities and/or health education activities for Alliance members

Establish and maintain effective working relationships with Provider offices, County departments and other community agencies related to care coordination for members, disease management,and/or health education

Recommend and implement program improvements that strengthen member access and health outcomes

ABOUT THE TEAM

We keep members healthy outside of the doctor's office through member support in prevention and disease management.

Knowledge of:

Principles and practices of managed health care and health care systems

Customer service

Health education principles and practices

Diverse needs of the Medi-Cal population

Ability to:

Understand and communicate the workings of the Alliance, particularly the Utilization Management and Complex Case Management, Community Care Coordination and Member Services departments

Use Microsoft Word and Excel, and Alliance computer systems

Verbally communicate the program mission, vision and roles

Education and Experience:

High school diploma or equivalent and three years (or an Associate's degree in health, social services or a related field or Medical Assistant certification and a minimum of two years) of relevant experience in a health care setting interacting with members, patients and/or providers in meeting service needs; or a combination of education and relevant work experience may be qualifying

OTHER DETAILS

Bilingual required: English/Spanish or English/Hmong

While this position is connected to one of our Alliance offices, we are in hybrid remote/in-office work environment right now and we anticipate that the interview process will take place remotely.

Our Alliance office locations have officially re-opened as of May 2, 2022 and while some employees may work in full-time telecommute schedules, attendance at quarterly company-wide events or department meetings will be expected.

Based on the nature of work, this position may require onsite presence, which is dependent on business need. Details about this can be reviewed during the interview process.

Scotts Valley pay range

$24.27

$38.83 USD

Merced pay range

$22.08

$35.34 USD

Salinas pay range

$24.27

$38.83 USD

Additionally, all positions at the Alliance are required to meet these minimum qualifications.
