Description
The Referral Coordinator 2 schedules and pre-registers patients for exams and procedures with specialists and providers outside of the primary care physician's office. The Referral Coordinator 2 performs varied activities and moderately complex administrative/operational/customer support assignments. Performs computations. Typically works on semi-routine assignments.
Responsibilities
The Referral Coordinator 2 gathers and communicates all relative information and preparation instructions to patient and referring providers. Decisions are typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.
Scheduling:
Ensures that requests for homecare service are filled promptly , and scheduled into computer in a timely manner
Ensure that homecare schedules are complete and accurate in computer
Reviews Santrax records daily to ensure that all staff are at clients as scheduled. Follows up immediately on any discrepancies
Reviews all scheduled homecare daily in preparation for payroll, and billing approvals by manager
Reports any client or staff issues to supervisor promptly
May participate in administrative on call backed up by professional staff for client/family issues
Managing overtime for caregivers
General Functions:
Participates in On-Call handoff meetings as necessary
Mandatory weekly meetings for Schedulers
Maintains on-call logs
Performs any other functions as requested by supervisor
In this role you will:
Schedule caregivers (CNAs & HHAs) for private pay home visits
Communicate scheduling changes with client families
Pull Santrax reports on a weekly basis
Participate in on-call as needed
Provide solid front line customer service to clients, caregivers and families
