The Field Care Manager Nurse 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager Nurse 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Responsibilities

The Field Care Manager, Behavioral Health (Care Manager, Behavioral Health RN) assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of member.

Position Responsibilities

The Field Care Manager, Behavioral Health Registered Nurse employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care.

Performs telephonic and face to face assessments and evaluations of the member's needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward the appropriate resources for the care and overall wellbeing of the member.

Ensures member is progressing towards desired outcomes by continuously monitoring care through assessments and/or evaluations.

Creates member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.

Collaborates with providers and community services to promote quality and cost-effective outcomes.

Ensures the delivery of needed services/supports for Behavioral Health, Physical Health, Social Determinant of Health and value added benefits.

Coordinates across the transdisciplinary care team (at a minimum the PCP) and transitions of care

Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Submits incident reports

Must reside in Louisiana

Territories that will be covered by multiple roles in the following:

Region 5 - Parishes -Calcasieu, Jefferson Davis. Cameron, Allen, Beauregard

Region 6 - Parishes - Grant, LaSalle, Catahoula, Avoyelles, Vernon, Winn, Rapides

Region 8 - Parishes - Union, Jackson, Morehouse, West Carrol, East Carrol, Madison, Richland, Tensas, Franklin, Ouachita, Lincoln, Caldwell

$5000 SIGN-ON BONUS for new associates! $3000 payable within the first 45 days and the remaining $2000 after 1 year of service.

Required Qualifications

Unrestricted Registered Nurse License in the State of Louisiana with at least 2 years experience in Behavioral Health or Addiction Recovery

Case management experience

Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel

Ability to use a variety of electronic information applications/software programs including electronic medical records

Exceptional communication and interpersonal skills with the ability to quickly build rapport

Ability to work with minimal supervision within the role and scope

Must reside in Louisiana

Preferred Qualifications

Case Management Certification (CCM) or willingness to obtain within two (2) years of employment

Experience working with Medicare, Medicaid and dual-eligible populations

Field Case Management Experience

Health Plan experience

Knowledge of community health and social service agencies and additional community resources

Experience with health promotion, coaching and wellness

Work-At-Home Requirements

Must have the ability to provide a high speed wireless, DSL or cable modem (including VPN tunnel) for a home office. Associates or contractors who live and work from home in the state of California will be provided payment for their internet expense.

A minimum standard speed for optimal performance of 25x10 (25mpbs download x 10mpbs upload) is required.

Satellite Internet service is NOT allowed for this role.

A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information

Workstyle: Combination remote work at home and onsite/home member visits

Training Date: 4/24/2023

Hours: Must be able to work a 40 hour work week, Monday through Friday 8:00 AM to 5:00 PM, over-time may be requested to meet business needs.

Travel: Must be willing to commute to meet with members in Louisiana - coverage areas for multiple positions:

Region 5 - Parishes -Calcasieu, Jefferson Davis. Cameron, Allen, Beauregard Region 6 - Parishes - Grant, LaSalle, Catahoula, Avoyelles, Vernon, Winn, Rapides

Region 8 - Parishes - Union, Jackson, Morehouse, West Carrol, East Carrol, Madison, Richland, Tensas, Franklin, Ouachita, Lincoln, Caldwell

COVID: Humana and its subsidiaries require vaccinated associates who work outside of their home to submit proof of vaccination, including COVID-19 boosters. Associates who remain unvaccinated must either undergo weekly negative COVID testing OR wear a mask at all times while in a Humana facility or while working in the field.

Driver's License, Transportation, Insurance

This role is a part of Humana's Driver Safety program and therefore requires and individual to have :

a valid state driver's license

proof of personal vehicle liability insurance with at least 100,000/300,000/100,00 limits,

and access to a reliable vehicle

Tuberculosis (TB) screening program

This role is considered patient facing and is part of Humana At Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
