The Social Services Resource Coordinator contributes to administration of care management the Primary Care team . Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well-being of members. The Resource Coordinator work assignments are often straightforward and of moderate complexity.

The Social Service Resource Coordinator employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care to fulfill referral needs. May collaborate to create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.
Humana's CenterWell organization is seeking a Social Service Resource Coordinator who works to support the clinical care model by employing a variety of strategies, approaches and techniques to manage a member's physical, environmental and psychosocial health issues. Social Service Resource Coordinator is responsible for screening, assessing and formulating resource recommendations for patients. The Social Service Resource Coordinator (LMSW) works as an integral part of the primary care team and makes decisions regarding own work methods, occasionally in ambiguous situations, requires minimal direction and receives guidance where needed. The main responsibilities of the role are:
Develops a comprehensive knowledge of Humana's Primary Care Organization strategy, best practices and tools.
Provides quality consultation with providers and staff regarding patient's SDOH and recommendations for appropriate resources and how it may be directly connected to the patient's medical conditions
Completes necessary documentation, which could include assessments, plans of care, and progress notes
Collaborates with and supports clinic level resource programs such as behavioral health specialists, care coach, and clinical pharmacist
Ensures adherence to defined policy, procedures and licensing standards based on federal, state, and local requirements
Assists leadership with value-based care initiatives, workflows and training of new Social Service Resource Coordinators
Required Qualifications
Bachelor's or Master's Degree in Social Work
Experience with Senior population and understanding of needs
Knowledge and experience in building local and state community resources networks
Strong MS office experience
EMR experience with strong documentation skills
Strong written and verbal communication skills
Strong experience in leading and facilitating group classes
Ability to work with various levels of interdisciplinary teams to achieve optimal patient care goals and measures
Proven ability to create and maintain health and trustworthy relationships
Adaptability to a fast paced and ever changing environment
Strong organizational skills
Flexibility to work from multiple locations based on patient and business needs
This role is considered patient facing and is part of Humana Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Valid state driver's license
Reliable and dependable transportation
Preferred Qualifications
Bilingual in English/Spanish
Previous Hospice, Home Health, Outpatient setting experience
