The Social Worker assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness by guiding members/families. The Social Worker's assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Responsibilities

Conviva Care Solutions is seeking a hard-working and dedicated Social Worker for the Broward County Region who is interested in being part of a team that focuses on excellent service to others.

The Social Worker employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues. Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Job Functions:

Assures that patients and families are knowledgeable of available community resources and how to access them.

Serves as a resource to clinical staff in dealing with complex psychosocial issues and accessing community resources.

When appropriate, provides direct Care coordination services to patients with complex social/emotional needs.

Documents patient/family status, diagnosis, treatment plan, goals and intervention, evaluation results, observations and progress in medical record.

Provides assistance with transportation services for medical visits.

Provides Food Bank referrals, Meals on Wheels referrals and other primary resources as needs are identified.

Provides education on supplemental services for Elder Care Assistance

Acts as a resource for materials on disease states, diet, general health and stress management.

Provides legal service referrals.

Provides assistance to complete referrals for supportive assistance at home

Serves as a community resource expert for local services and support groups.

Provides support to Care Coaches and Conviva Providers as needed to coordinate services listed above

Refer complex patient concerns to the member's physician and medical director for appropriate interventions.

Works closely with the hospital case managers, attending, PCP and medical director, community agencies and other approved vendors to ensure the member's healthcare needs are met.

Acts as a resource and is able to facilitate eligibility and benefit information.

Maintains accurate and concise documentation and adheres to documentation guidelines.

Adheres to the Conviva Care Solutions policies and procedures.

Maintains confidentiality and adheres to HIPAA guidelines at all times.

Participate in high-risk member meetings.

Qualifications/Requirements:

Bachelor's degree in social work (Master in Social work preferred, not required)

1 years of post-graduate social work experience

Case management experience

Knowledge of community programs/services

Discharge planning experience

Managed care experience

Computer proficiency: Internet, Microsoft Word, Microsoft Excel, Email

Excellent written and verbal communication skills

Effective interpersonal skills required

Excellent customer service. Must interact with patients in a respectful and amicable manner.

Team members are expected to treat patients with compassion and protect their rights

Adhere to all Company Policies

Answer telephones

Assist in compiling statistics

Attends and participates in meetings

Maintain logs

PC laptops

Preferred Qualifications

Bilingual a plus (English/Spanish)

Social Work License (Florida only licenses for Masters degree level Social Workers. License N/A for Bachelor's Degree

Your recorded interview(s) via text and/or pre-recorded voice will be reviewed and you will subsequently be informed if you will be moving forward to next round of interviews.
